基于"肾亏毒滞"与氧化应激相关性探讨狼疮性肾炎中医辨治OA
Exploration of TCM treatment based on syndrome differentiation of lupus nephritis based on the correlation between kidney deficiency and toxin stagnation and oxidative stress
氧化应激是指氧化与抗氧化系统失衡导致细胞内活性氧积聚引发损伤的过程.狼疮性肾炎(LN)发病与氧化应激相关.基于"肾亏毒滞"理论辨治LN关键病机为"肾虚伏毒,毒损肾络",发病之本为肾阴亏损、邪毒潜伏,病变之因为毒聚热盛、瘀阻肾络,病笃之基为因损致虚、阴阳两虚.提出以氧化应激为干预靶点分期治疗:潜伏期杜邪截源以抗氧化应激、调免疫,发病期即病防变以清病理产物、抑损伤,加重期攻补兼施以改善微环境、复稳态,具体治法及遣方用药需整体审机、循证立方.
Oxidative stress refers to the process in which the imbalance between oxidative and antioxidant systems leads to damage caused by the accumulation of reactive oxygen species within cells.Lupus nephritis(LN)pathogenesis is associated with oxidative stress.Based on the theory"kidney deficiency and toxin stagnation",the key pathogenesis for treatment based on syndrome differentiation of LN is"kidney deficiency harboring toxins,toxins damaging kidney meridian".Its fundamental cause is the loss of kidney yin and latent toxins,the pathological reason is toxin accumulation with excessive heat and blood stasis obstructing kidney collaterals,and the root of the critical condition is deficiency caused by damage,leading to dual deficiency of yin and yang.The staged treatment targeting oxidative stress as an intervention point has been proposed,including the latent phase requiring pathogen elimination and source interception to resist oxidative stress and regulate immunity,the onset phase requiring immediate disease prevention and transformation to clear pathological products and suppress damage,and the exacerbation phase requiring both attack and tonify to improve the microenvironment and restore homeostasis.Specific treatment methods and prescription of medications require comprehensive assessment of the underlying mechanisms and evidence-based formulation.
