目的 探讨机器人辅助腹腔镜单一体位根治性肾输尿管全长加膀胱袖状切除术(robot-assisted radical nephroureterectomy with bladder cuff excision,RARNU-BCE)治疗上尿路尿路上皮癌(upper tract urothelial carcinoma,UTUC)的安全性、可行性及临床疗效.方法 回顾性分析 2023 年 1 月至 2025 年 6 月于滨州医学院附属医院接受达芬奇Xi机器人系统辅助下"一体位"RARNU-BCE治疗的 8 例UTUC患者(机器人组)的临床资料.所有手术均由同一术者完成,患者取改良侧卧位,采用特定套管布局(如"闪电"布局),在不重新摆位或重新对接机器人的情况下一次性完成肾脏游离、输尿管全长切除、淋巴结清扫及膀胱袖状切除.记录并分析患者的手术时间、术中出血量、术后并发症、住院时间、病理结果及短期随访结果.结果 8 例手术均顺利完成,无中转开放手术.手术时间(233.25±66.5)min,术中出血量(175±46.29)mL.术后无严重并发症(Clavien-Dindo≥III级)发生.术后病理诊断为UTUC,切缘均为阴性.术后随访2~6 个月,所有患者均无肿瘤复发或转移迹象.结论 机器人辅助腹腔镜单一体位RARNU-BCE治疗UTUC具有手术视野清晰、操作精准、术中出血少、术后恢复快等优点,是一种安全、可行且有效的微创手术方式.其"一体化"流程避免了术中再次摆位,简化了手术步骤,具有减少手术创伤、缩短总手术时间的潜力,近期肿瘤控制效果满意.

Objective To evaluate the safety,feasibility,and clinical efficacy of single-position robot-assisted laparoscopic radical nephroureterectomy with bladder cuff excision(RNU-BCE)in the treatment of upper tract urothelial carcinoma(UTUC).Methods The clinical data of 8 UTUC patients who underwent single-position RARNU-BCE using the Da Vinci Xi robotic sys-tem in Binzhou Medical University Hospital from January 2023 to June 2025(robot-assisted group)were retrospectively ana-lyzed.All surgeries were performed by the same surgeon with the patient in a modified lateral decubitus position.A specific tro-car configuration(e.g.,"lightning"layout)was used,and the entire procedure,including kidney mobilization,complete ure-terectomy,lymph node dissection,and bladder cuff excision,was completed in a single position without the need for patient repo-sitioning or robot redocking.Operative time,intraoperative blood loss,postoperative complications,hospital stay,pathological results,and short-term follow-up outcomes were recorded and analyzed.Results All 8 procedures were successfully completed without conversion to open surgery.The mean operative time was(233.25±66.5)minutes,and the mean intraoperative blood loss was(175±46.3)mL.No major postoperative complications(Clavien-Dindo grade≥III)occurred.Postoperative pathology confirmed UTUC in all cases with negative surgical margins.During a follow-up period of 2-6 months,no signs of tumor recur-rence or metastasis were observed in any patient.Conclusion Single-position robot-assisted laparoscopic RNU-BCE for UTUC offers advantages such as a clear surgical field,precise manipulation,minimal intraoperative bleeding,and rapid postoperative recovery.It is a safe,feasible,and effective minimally invasive surgical technique.The integrated procedure eliminates the need for intraoperative repositioning,simplifies surgical steps,and may potentially reduce surgical trauma and total operative time.Short-term tumor control outcomes are satisfactory.
