Optimization of mild-warm moxibustion parameters for premature ovarian insufficiency based on the orthogonal design and multi-model comprehensive evaluation

目的 基于正交设计与多模型综合评价观察不同灸量参数组合温和灸对早发性卵巢功能不全(POI)小鼠卵巢功能的整体效应.方法 采用 L9(34)正交表设计,以灸时(因素 A)、灸距(因素B)、灸频(因素 C)为 3 个因素,每个因素设 3 个水平,形成 9 个不同灸量参数组合温和灸干预组,即Ⅰ组~Ⅸ组.采用随机数字表法将SPF 级雌性C57BL/6J 小鼠分至各干预组,另设空白组与模型组,每组15 只.除空白组外,采用雷公藤多苷(75 mg/kg)连续灌胃14 d 制备 POI 模型.各干预组按正交表设定的参数,隔次交替温和灸双侧"肾俞"或"中脘""关元",连续 14 d.采用阴道细胞涂片观察小鼠动情周期紊乱情况;酶联免疫吸附测定法检测小鼠血清雌二醇(E2)、卵泡刺激素(FSH)、黄体生成素(LH)、抗米勒管激素(AMH)、睾酮(T)含量;HE 染色法观察小鼠卵巢组织形态并进行各级卵泡计数;合笼观察并计算小鼠妊娠率、着床数、活胎数及着床后死亡率.将动情周期紊乱率和 FSH 含量设为主要疗效指标,其余指标为次要疗效指标,通过极差分析、方差分析解析单指标效 应,指标相关性权法(CRITIC)与熵权法确定各指标权重,结合逼近理想解排序法(TOPSIS)、灰色关联度分析进行综合评价,优选温和灸灸量参数.结果 与空白组比较,模型组动情周期紊乱率上升,血清 E2、AMH 含量下降,FSH、LH、T 含量升高,卵巢原始卵泡数、初级卵泡数、次级卵泡数、窦状卵泡数减少,闭锁卵泡数增加,妊娠率、着床数、活胎数减少,着床后死亡率增加(P<0.05).与模型组比较,各干预组指标均有不同程度改善.综合正交试验、CRITIC-TOPSIS 法、熵权-TOPSIS 法与灰色关联度分析筛选出Ⅳ组(灸时 10 min、灸距 1 cm、1 次/d)为优选灸量参数组合.该组可降低动情周期紊乱率,增加血清E2、AMH 含量,降低FSH、LH、T 含量,增加原始卵泡数、初级卵泡数、次级卵泡数,减少闭锁卵泡,提高妊娠率、着床数、活胎数(P<0.05).结论 灸时10 min、灸距1 cm、1 次/d 的灸量参数组合是温和灸改善POI 模型小鼠卵巢功能与生育能力的优选方案.

Objective The overall effect of mild-warm moxibustion on ovarian function in mice with premature ovarian insufficiency(POI)was observed by orthogonal design and multi-model comprehensive evaluation with different moxibustion parameters(MP).Methods The experiment was conducted using an L9(34)orthogonal array,with duration(factor A),distance(factor B),and frequency(factor C)as the three factors,each set at three levels.This arrangement resulted in mild-warm moxibustion intervention groups Ⅰ-Ⅸ.SPF female C57BL/6J mice were randomly assigned to these groups using a random number table method,including a blank and model group,with 15 mice per group.The POI model was established via continuous gavage of Tripterygium wilfordii multiglucoside(75 mg/kg)for 14 days.In accordance with the MP specified in the orthogonal table,each group received alternating mild-warm moxibustion on both sides of the"Shenshu"(BL23),or"Zhongwan"(CV12)and"Guanyuan"(CV4)acupoints every other session for 14 days.The estrous cycle disorder of the mice was monitored by vaginal smear.Serum estradiol(E2),follicle-stimulating hormone(FSH),luteinizing hormone(LH),anti-Müllerian hormone(AMH),and testosterone(T)levels were measured using an enzyme-linked immunosorbent assay.Ovarian morphology and follicle count were assessed using hematoxylin-eosin staining.The pregnancy rate,implantation count,live fetuses,and post-implantation mortality rate were evaluated via cohabitation observation.The estrous cycle disorder rate and FSH level were defined as the primary efficacy indicators,with the remaining indicators serving as secondary indicators.The effects of single indicators were analyzed through range and variance analyses.The Criteria Importance Through Intercriteria Correlation(CRITIC)method and the entropy weight method were used to determine indicator weights,which were then combined with the Technique for Order Preference by Similarity to Ideal Solution(TOPSIS)and grey relational analysis for comprehensive evaluation to identify the preference MP.Results Compared with the blank group,the model group showed an increase in the estrous cycle disorder rate,decreased serum E2 and AMH levels,increased FSH,LH,and T levels,reduced numbers of primordial,primary,secondary,and antral follicles,increased numbers of atretic follicles,decreased pregnancy rate,implantation count,and number of live fetuses,and increased post-implantation mortality(P<0.05).Compared with the model group,all intervention groups showed varying degrees of improvement in the indicators mentioned above.The orthogonal experiment,CRITIC-TOPSIS,entropy weight-TOPSIS,and grey relational analysis identifiedⅣ group(10 min duration,1 cm distance,once daily)as the preference parameter combination.This combination significantly reduced the estrous cycle disorder rate,upregulated serum E2 and AMH levels,downregulated FSH,LH,and T levels,increased the numbers of primordial,primary,and secondary follicles,decreased the number of atretic follicles,and improved the pregnancy rate,implantation count,and number of live fetuses(P<0.05).Conclusion The preferred MP for treating POI is 10 min duration,1 cm distance,and once daily,which improves ovarian function and fertility in POI mice.
