青少年近视人群角膜体积分布特点及与眼球生物参数相关性分析OA
Analysis of corneal volume distribution characteristics and correlation with eye biological parameters in adolescents with myopia
目的 探究青少年近视人群角膜体积(corneal volume,CV)分布特点及与眼球生物参数的相关性.方法 采取随机分层抽样方法,收集2023年9月—2024年3月在成都市第三人民医院进行近视检查的青少年患者110例,根据年龄段分为低龄组(10~17岁,n=46)和高龄组(18~24岁,n=64);根据近视程度分为轻度近视组(SE<-3.00D,n=52)、中度近视组(-3.00D≤SE≤-6.00D,n=36)和高度近视组(SE>-6.00D,n=22).以角膜顶点为中心,测量直径为3 mm、5 mm、7 mm的角膜区域.比较不同年龄、不同近视程度青少年近视患者眼球生物参数及CV.通过Spearman相关性分析来评估CV与其他眼部生物学指标之间的关联性.采用限制性立方样条(restricted cubic spline,RCS)拟合回归模型探究其他眼部生物学参数与CV的关系.利用P值及检验立方项参数的Pn值来判断带有立方项的非线性模型是否有统计学意义.结果 高龄组的角膜曲率、眼轴长度、屈光度、角膜曲率半径及CV-3 mm、CV-5 mm、CV-7 mm均高于低龄组,晶体厚度低于低龄组(P<0.05).中度近视组和高度近视组患者的角膜曲率、眼轴长度、屈光度、角膜曲率半径及CV-3 mm、CV-5 mm、CV-7 mm均高于轻度近视组(P<0.05).Spearman的相关分析结果显示,CV-3 mm、CV-5 mm、CV-7 mm与中央角膜厚度呈显著正相关关系(rCV-3 mm=0.985,rCV-5 mm=0.976,rCV-7 mm=0.956).限制性立方样条分析结果显示,只有角膜曲率和角膜曲率半径与CV-3 mm、CV-5 mm、CV-7 mm存在非线性关系(P和Pn均小于0.05),其余眼球生物学参数指标与CV-3 mm、CV-5 mm、CV-7 mm均不存在非线性关系.结论 随着年龄和近视程度的增加,青少年角膜曲率、眼轴长度和CV增大,晶体厚度减小,且CV与中央角膜厚度正相关,角膜曲率与CV呈非线性关系.
Objective To explore the distribution characteristics of corneal volume(CV)and its correlation with ocular biological parameters in adolescents with myopia.Methods A total of 110 adolescent patients who underwent myopia examination in The Third People's Hospital of Chengdu from September 2023 to March 2024 were collected by random stratified sampling,and were divided into the younger age group(10-17 years,n=46)and the older age group(18-24 years,n=64)according to their age groups.According to the degree of myopia,they were divided into mild myopia group(SE<-3.00D,n=52),moderate myopia group(-3.00D≤SE≤-6.00D,n=36)and high myopia group(SE>-6.00D,n=22).Centered on the corneal apex,the corneal region with diameters of 3 mm,5 mm,and 7 mm was measured.The biological parameters of the eyeballs and CV of adolescent myopia patients of different ages and degrees of myopia were compared.Spearman correlation analysis was used to evaluate the association between CV and other ocular biological indicators.The restricted cubic spline(RCS)fitting regression model was used to explore the relationship between other ocular biological parameters and CV.The P-value and the Pn value of the test cube term parameters were used to determine whether the nonlinear model with the cube term was statistically significant.Results The corneal curvature,axial length,refraction,radius of corneal curvature,CV-3 mm,CV-5 mm,and CV-7 mm in the older age group were higher than those in the younger age group,and the lens thickness was lower than that in the younger age group(P<0.05).The corneal curvature,axial length,refraction,radius of corneal curvature,CV-3 mm,CV-5 mm,and CV-7 mm in the moderate myopia group and the high myopia group were higher than those in the mild myopia group(P<0.05).Spearman's correlation analysis showed that CV-3 mm,CV-5 mm,and CV-7 mm were significantly positively correlated with central corneal thickness(rCV-3 mm=0.985,rCV-5 mm=0.976,rCV-7 mm=0.956).The results of RCS analysis showed that only the corneal curvature and corneal curvature radius had a nonlinear relationship with CV-3 mm,CV-5 mm,and CV-7 mm(P and Pn<0.05),and other ocular biological parameters had no nonlinear relationship with CV-3 mm,CV-5 mm,and CV-7 mm.Conclusions With the increase of age and myopia,the corneal curvature,axial length and CV of adolescents increased,and the lens thickness decreased,and the CV was positively correlated with the central corneal thickness,and the corneal curvature had a nonlinear relationship with CV.
