从"气火失衡"论治重症肺炎OA

Severe pneumonia(SP)presents with critical conditions and a high mortality rate,making it a major challenge in the field of modern respiratory critical care.Although modern medical supportive therapies centered on anti-infection have advanced,they still face challenges such as drug resistance and organ function preservation.Traditional Chinese Medicine(TCM),based on its holistic view and syndrome differentiation,demonstrates unique value in synergistically improving patients'systemic status and prognosis.This article takes Li Gao's theory of"fire and primordial qi cannot coexist"as its core to systematically elucidate the pathogenesis,treatment principles,and stage-based management of SP.The study proposes that the dynamic imbalance between"primordial qi(yuan qi)and yin fire"is the core pathogenesis of SP.Deficiency of the spleen and stomach constitutes the root cause,leading to the internal generation of yin fire.The endogenous yin fire consumes fluids and blood,further producing pathological products such as phlegm,blood stasis,and fluid retention.These pathogenic factors intermingle and ascend to disturb the lung,creating a vicious cycle.Based on this,a stage-based treatment principle is established:in the acute phase,both the root and the manifestation should be addressed concurrently.The treatment should focus on"lifting yang and dissipating fire",aiming to reinforce the spleen and stomach's middle qi while clearing and draining yin fire.In the remission phase,the focus shifts to treating the root cause by supplementing the spleen and boosting qi,thereby consolidating the foundation and preventing recurrence through the principle of"fortifying the earth to generate metal".A case study included at the end demonstrates the dynamic diagnostic and treatment process,transitioning from draining yin fire to supplementing middle qi,verifying the clinical applicability of this theory.The"qi-fire imbalance"staging framework constructed in this study provides a classic-originated,logically coherent new perspective for the TCM diagnosis and treatment of severe pneumonia.It holds theoretical significance for enriching the syndrome differentiation system of lung diseases,though its clinical application value requires further validation through more in-depth research.
