目的:探讨不同时序针灸结合康复机器人辅助步态训练(RAGT)治疗帕金森病(PD)的临床疗效.方法:将60例PD患者随机分为对照组(30例)和观察组(30例),对照组接受针刺百会、大椎结束后进行RAGT,观察组在留针状态下同步进行RAGT,两组均治疗12周.于治疗前及治疗第4周、第8周、第12周评估两组统一帕金森病评定量表(UPDRS)评分、6 min步行试验(6MWT)、39项帕金森病生活质量问卷(PDQ-39)评分.结果:对照组UPDRS总分在治疗8周和12周后较治疗前显著降低(P<0.05),该改善主要源于UPDRS-III量表评分下降,而UPDRS-I、II、IV评分无显著变化;观察组UPDRS总分在各时间点均较治疗前显著下降(P<0.05),改善范围涵盖UPDRS-I、II、III评分;此外,两组6MWT距离与PDQ-39评分在各时间点均较治疗前改善(P<0.05).组间比较显示,观察组在治疗8周和12周时的UPDRS-III评分显著低于对照组(P<0.05),治疗12周时UPDRS总分亦显著更低(P<0.05);但两组间的6MWT距离及PDQ-39评分在各时间点均无统计学差异(P>0.05).结论:针刺百会、大椎结合RAGT可有效改善PD患者的UPDRS评分、步行能力和生活质量;与先针灸后康复训练相比,留针同步训练在改善患者运动功能方面显示出更好的远期效果.

Objective:To explore the clinical efficacy of different temporal sequences of acupuncture combined with rehabilitation robot-assisted gait training(RAGT)in the treatment of Parkinson's disease(PD).Methods:Sixty PD patients were randomly divided into a control group(30 cases)and an observation group(30 cases).The control group received RAGT after acupuncture at Baihui(GV20)and Dazhui(GV14),while the observation group received RAGT simultaneously with acupuncture.Both groups were treated for 12 consecutive weeks.The Unified Parkinson's Disease Rating Scale(UPDRS)score,6-minute walk test(6MWT)distance,and 39-item Parkinson's Disease Questionnaire(PDQ-39)score were evaluated in the two groups before treatment,at the 4th week,the 8th week,and the 12th week.Results:After 8 and 12 weeks of treatment,the total UPDRS score of the control group was significantly lower than that before treatment(P<0.05).The improvement was mainly due to the decrease in the UPDRS-III scale score,while the UPDRS-I,II,and IV scores showed no significant changes(P>0.05).The total UPDRS score of the observation group was significantly lower than that before treatment at each time point(P<0.05),and the improvement range covered the UPDRS-I,II,and III scores.In addition,compared with before treatment,the 6MWT distance was increased and the PDQ-39 score was decreased in both groups at each time point(P<0.05).At the 8th and 12th weeks of treatment,the UPDRS-III score of the observation group was significantly lower than that of the control group(P<0.05);at the 12th week of treatment,the total UPDRS score of the observation group was also significantly lower than that of the control group(P<0.05).However,there was no statistically significant difference in the 6MWT distance and PDQ-39 score between the two groups at each time point(P>0.05).Conclusion:Acupuncture at Baihui(GV20)and Dazhui(GV14)combined with RAGT can effectively improve the UPDRS score,walking ability,and quality of life of PD patients.Compared with acupuncture first followed by rehabilitation training,synchronous training with needle retention has a better long-term effect in improving the motor function of patients.
