目的 探讨体位辅助固定器在早产儿上肢经外周静脉置入中心静脉导管(peripherally inserted central catheter,PICC)胸部X线尖端定位中的临床应用效果.方法 选取2024年4月至9月于浙江省嘉兴市妇幼保健院接受上肢PICC置管的60例早产儿作为对照组,2024年10月至2025年4月接受上肢PICC的60例早产儿作为试验组.两组早产儿在PICC置管术后均行胸部X线摄片进行尖端定位,对照组采用小沙袋固定体位后摄片,试验组采用体位辅助固定器固定体位后摄片,比较两组患儿导管尖端定位准确一次摄片成功率、摄片时体位固定时间、早产儿生理指标稳定性.结果 试验组早产儿导管尖端定位准确一次摄片成功率高于对照组,试验组早产儿摄片时体位固定用时少于对照组,试验组早产儿生理指标稳定性明显优于对照组,差异均有统计学意义(P<0.05).结论 体位辅助固定器用于早产儿上肢PICC胸部X线尖端定位可明显提高导管尖端定位准确的一次成功率并可缩短摄片时用于固定体位的时间,提高早产儿生理指标的稳定性.

Objective To explore the effectiveness of a postural assisted fixator in tip positioning of peripherally inserted central catheters(PICC)on the chest of premature infants.Methods A total of 60 preterm infants who underwent upper limb PICC catheterization in Jiaxing Maternal and Child Health Hospital from April to September 2024 were selected as control group,while another 60 preterm infants who received upper limb PICC catheterization from October 2024 to April 2025 were assigned to experimental group.Both groups underwent chest X-ray imaging for catheter tip positioning.The control group was positioned with small sandbags for imaging,whereas the experimental group used a postural assisted fixator.The accuracy of catheter tip positioning,the success rate of single-image positioning,the duration of position fixation during imaging,and the stability of physiological parameters in preterm infants were compared between two groups.Results The success rate of one radiographic exposure with accurate catheter tip positioning in experimental group of preterm infants was higher than that in control group.The time required for position fixation during radiography in experimental group was shorter than that in the control group.The stability of physiological indicators in the experimental group was significantly better than that in control group,with all differences being statistically significant(P<0.05).Conclusion The use of postural assisted fixator for tip positioning of PICC on the chest of premature infants can significantly improve the success rate of catheter tip positioning on the first attempt and reduce the time required for positioning during radiography,thereby enhancing the stability of physiological parameters in premature infants.
