肥胖患儿代谢功能障碍相关脂肪性肝病患病因素分析OA
Analysis of risk factors of metabolic dysfunction-associated steatotic liver disease in obese children
目的 探讨临床指标和稳态模型胰岛素抵抗指数(homeostasis model of assessment for insulin resistance index,HOMA-IR)水平在肥胖患儿代谢功能障碍相关脂肪性肝病(metabolic dysfunction-associated steatotic liver disease,MASLD)中的变化及其临床意义.方法 选取2023年5月至2025年9月苏州大学附属第三医院常州市第一人民医院收治的138例肥胖患儿作为研究对象.根据是否伴发MASLD分为单纯肥胖组(n=79)与MASLD组(n=59).比较两组患儿的各项临床指标,采用多因素Logistic回归性分析肥胖患儿MASLD的危险因素.结果 MASLD组患儿的年龄及丙氨酸转氨酶、天冬氨酸转氨酶、甘油三酯、25-羟维生素D、HOMA-IR显著高于单纯肥胖组(P<0.05).Logistic回分析结果表明,HOMA-IR是肥胖患儿发生MASLD的独立危险因素.结论 当肥胖患儿HOMA-IR水平明显升高时,应监测评论MASLD发生风险.
Objective To explore the variation patterns and clinical significance of clinical indicators and homeostasis model assessment for insulin resistance index(HOMA-IR)in metabolic dysfunction-associated steatotic liver disease(MASLD)in obese children.Methods A total of 138 obses pediatric patients admitted to the Third Affiliated Hospital of Soochow University,Changzhou First People's Hospital from May 2023 to September 2025 were selected as the study subjects.Based on the presence or absence of MASLD,the patients were divided into simple obesity group(n=79)and MASLD group(n=59).Clinical indicators of two groups were compared,and multivariate Logistic regression analysis was performed to identify risk factors for MASLD in obese children.Results The age and levels of alanine aminotransferase,aspartate aminotransferase,triglycerides,25-hydroxyvitamin D,and HOMA-IR in MSLD group were significantly higher than those in simple obesity group(P<0.05).Logistic regression analysis revealed that HOMA-IR was an independent risk factor for the development of fatty liver in obese children.Conclusion When the HOMA-IR level of obese children is significantly elevated,the risk of MASLD should be monitored and evaluated.
