阴道彩超下滋养细胞血流信号在剖宫产瘢痕妊娠早期诊断中的应用研究OA

Objective:To explore the application value of trophoblast blood flow signals under transvaginal color Doppler ultrasound in the early diagnosis of cesarean scar pregnancy(CSP).Method:A retrospective analysis was conducted on the data of 70 suspected CSP patients admitted to Shangrao Maternal and Child Health Care Hospital from June 2021 to June 2023.All patients underwent transvaginal color Doppler ultrasound examination before the operation,and their ultrasound image manifestations were recorded.Taking the surgical pathological results as the gold standard,the application value of trophoblast blood flow signals under transvaginal color Doppler ultrasound in the early diagnosis of CSP was analyzed.Result:Among the 70 patients,61 cases were diagnosed with CSP through surgical pathological,accounting for 87.14%.Totally 56 cases of CSP were confirmed by transvaginal color Doppler ultrasound,including 26 cases of mass type,18 cases of gestational sac type,and 12 cases of residual type.Taking surgical pathological diagnosis as the gold standard,the sensitivity of trophoblast blood flow signals under transvaginal color Doppler ultrasound in diagnosing CSP was 90.16%(55/61),the specificity was 88.89%(8/9),the accuracy rate was 90.00%(63/70),and the consistency with surgical pathological diagnosis was relatively high,with a Kappa value of 0.639.Transvaginal color Doppler ultrasound showed that trophoblast blood flow signals of the vast majority of CSP patients(86.89%)originated from the scar.Conclusion:The diagnostic value of trophoblast blood flow signals under transvaginal color Doppler ultrasound for CSP was relatively high,and they also demonstrate certain clinical value in classification of CSP,which can provide a reference for the early diagnosis and treatment of CSP.
