目的:评估血液灌流(HP)联合血液透析(HD)治疗尿毒症患者的临床效果,着重探讨其对肾功能、炎症因子及钙磷代谢参数的影响.方法:前瞻性纳入 2022 年 2 月—2024 年 2 月抚州市第一人民医院肾脏内科收治的 84 例尿毒症患者,按随机数字表法分为观察组(行 HP 联合 HD 治疗,n=42)与对照组(仅行 HD 治疗,n=42).对比两组治疗前、治疗 3 个月后的肾功能指标、炎症因子及钙磷代谢相关指标的变化,并统计不良反应发生率.结果:治疗 3 个月后,观察组 BUN 及 Scr 水平明显低于对照组(P<0.05).治疗 3 个月后,观察组血清 CRP、IL-6、TNF-α 水平均显著低于对照组(P<0.05).治疗3 个月后,观察组血磷、全段甲状旁腺素(iPTH)水平均显著低于对照组(P<0.05).两组不良反应发生率比较,差异无统计学意义(P>0.05).结论:HP 联合 HD 通过协同清除机制显著优化尿毒症患者的肾功能,抑制微炎症反应,并纠正钙磷代谢紊乱,且不增加治疗风险.

Objective:To evaluate the clinical effect of hemoperfusion(HP)combined with hemodialysis(HD)in the treatment of uremia patients,with emphasis on exploring its influence on renal function,inflammatory factors and calcium and phosphorus metabolism parameters.Method:A total of 84 patients with uremia admitted to the Department of Nephrology of Fuzhou First People's Hospital from February 2022 to February 2024 were prospectively included and divided into the observation group(treated with HP combined with HD,n=42)and the control group(treated only with HD,n=42)according to the random number table method.The changes of renal function indicators,inflammatory factors and calcium and phosphorus metabolism related indicators before treatment and after 3 months of treatment in the two groups were compared,and the incidence of adverse reactions was statistically analyzed.Result:After 3 months of treatment,the levels of BUN and Scr in the observation group were significantly lower than those in the control group(P<0.05).After 3 months of treatment,the levels of serum CRP,IL-6 and TNF-α in the observation group were significantly lower than those in the control group(P<0.05).After 3 months of treatment,the levels of blood phosphorus and intact parathyroid hormone(iPTH)in the observation group were significantly lower than those in the control group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:HP combined with HD significantly optimizes the renal function of uremia patients,inhibits micro-inflammatory responses and corrects calcium and phosphorus metabolism disorders through a synergistic clearance mechanism,without increasing the treatment risk.
