独活寄生汤加减联合中药塌渍治疗肝肾亏虚型腰椎间盘突出症的临床效果OA
Clinical Effect of Modified Duhuo Jisheng Decoction and Chinese Herbal Hot Compress in the Treatment of Lumbar Disc Herniation of Liver and Kidney Deficiency Type
目的:探讨在肝肾亏虚型腰椎间盘突出症(LDH)患者中应用独活寄生汤加减联合中药塌渍治疗的临床效果.方法:选取 2023 年 8 月—2024 年 8 月洪湖市中医医院收治的 96 例肝肾亏虚型 LDH患者,以随机数字表法分为研究组(n=48)与对照组(n=48).对照组实施西医常规治疗,研究组在对照组基础上实施独活寄生汤加减联合中药塌渍治疗,两组均治疗 4 周.比较两组临床疗效、中医证候积分、腰椎功能、疼痛程度、血清指标.结果:研究组治疗总有效率为95.83%,高于对照组的81.25%(P<0.05).治疗后,两组中医证候积分较治疗前降低,且研究组较对照组低(P<0.05).治疗后,两组日本骨科学会(JOA)腰痛评分较治疗前升高,改良 Oswestry 功能障碍指数(ODI)、数字评分法(NRS)评分较治疗前降低,且研究组 JOA 腰痛评分较对照组高,改良 ODI、NRS 评分较对照组低(P<0.05).治疗后,两组IL-1β、转化生长因子-β1(TGF-β1)、血栓烷 B2(TXB2)水平较治疗前下降,且研究组较对照组低(P<0.05).结论:肝肾亏虚型 LDH 患者采用独活寄生汤加减联合中药塌渍治疗能够提高临床效果和腰椎功能,降低中医证候积分、疼痛程度,改善血清指标水平.
Objective:To explore the clinical effect of the modified Duhuo Jisheng Decoction combined with Chinese herbal hot collapse in the treatment of patients with lumbar disc herniation(LDH)of liver and kidney deficiency type.Method:A total of 96 liver and kidney deficiency type LDH patients from August 2023 to August 2024 who were admitted in Honghu Hospital of Traditional Chinese Medicine were selected,and divided into the study group(n=48)and the control group(n=48)by the random number table method.The control group received conventional western medical treatment,while the study group was treated with modified Duhuo Jisheng Decoction combined with Chinese herbal hot compress on the basis of the control group.Both groups were treated for 4 weeks.The clinical efficacy,traditional Chinese medicine syndrome score,lumbar function,pain degree and serum indicators of the two groups were compared.Result:The total effective rate of treatment in the study group was 95.83%,which was higher than 81.25%in the control group(P<0.05).After treatment,the traditional Chinese medicine syndrome scores of both groups were lower than those before treatment,and those of the study group were lower than those of the control group(P<0.05).After treatment,the Japanese Orthopaedic Association(JOA)low back pain scores of both groups increased compared with those before treatment,while the modified Oswestry disability index(ODI)and numerical rating scale(NRS)scores decreased compared with those before treatment;moreover,the JOA low back pain score of the study group was higher than that of the control group,and the modified ODI and NRS scores were lower than those of the control group(P<0.05).After treatment,the levels of IL-1β,transforming growth factor-β1(TGF-β1),and thromboxane B2(TXB2)in both groups decreased compared with those before treatment,and the levels in the study group were lower than those in the control group(P<0.05).Conclusion:For patients with LDH of liver and kidney deficiency type,the treatment with modified Duhuo Jisheng Decoction combined with Chinese herbal hot compress can improve the clinical effect and lumbar function,reduce the traditional Chinese medicine syndrome score and pain degree,and improve the serum index levels.
