经鼻高流量氧疗不同流速对慢性阻塞性肺疾病患者的影响OA
Effects of Different Flow Rates of High-Flow Nasal Cannula on Patients with Chronic Obstructive Pulmonary Disease
目的:探讨经鼻高流量氧疗(high-flow nasal cannula,HFNC)不同流速对慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者膈肌功能、舒适度、呼吸窘迫度及动脉血气指标的影响.方法:纳入 2023 年 12 月—2024 年 6 月蚌埠医科大学第一附属医院收治的 90 例COPD患者为研究对象,随机分为低流速组(30 L/min)和高流速组(60 L/min),每组 45 例,分别于治疗前及治疗 2、8、24 h后通过床旁超声测量患者的平静呼吸时膈肌移动度(DEq)及膈肌浅快呼吸指数(D-RSBI),同时记录患者的舒适度、呼吸窘迫度及动脉血气指标.结果:治疗前两组DEq、D-RSBI、舒适度、呼吸窘迫度、PaO2、PaCO2 比较,差异无统计学意义(P>0.05).治疗 2、8、24 h后高流速组DEq、D-RSBI均低于低流速组(P<0.05);治疗 2、8、24 h后低流速组舒适度、呼吸窘迫度均优于高流速组(P<0.05);治疗2、8、24 h后两组PaO2、PaCO2 比较差异无统计学意义(P>0.05).结论:HFNC不同流速设置对COPD患者膈肌功能、生理指标及主观舒适度有显著的影响,在优化生理效益的同时,应兼顾患者的舒适度,以实施个体化氧疗.
Objective:To investigate the impact of different flow rates of high-flow nasal cannula(HFNC)on diaphragmatic function,subjective comfort,respiratory distress,and arterial blood gas parameters in patients with chronic obstructive pulmonary disease(COPD).Method:Ninety COPD patients admitted to the First Affiliated Hospital of Bengbu Medical University from December 2023 to June 2024 were enrolled and randomly assigned to either a low-flow rate group(30 L/min)and a high-flow rate group(60 L/min),with 45 patients in each group.Diaphragmatic excursion during quiet breathing(DEq)and diaphragmatic rapid shallow breathing index(D-RSBI)were assessed using bedside ultrasound before treatment and at 2,8 and 24 hours after intervention.Simultaneously,comfort level,degree of respiratory distress,and arterial blood gas values of patients were recorded.Result:No significant differences were observed between groups before treatment in DEq,D-RSBI,comfort level,degree of respiratory distress,PaO2,and PaCO2(P>0.05).At 2,8,24 h after treatment,the DEq and D-RSBI values of the high-flow rate group were lower than those of the low-flow rate group(P<0.05);at 2,8,24 h after treatment,the comfort level,and degree of respiratory distress of the low-flow rate group were better than those of the high-flow rate group(P<0.05).At 2,8,24 h after treatment,PaO2 and PaCO2 values did not differ significantly between the two groups(P>0.05).Conclusion:HFNC at different flow rates significantly influences diaphragmatic function,physiological parameters,and patient-perceived comfort in COPD patients.To maximize therapeutic benefit,individualized oxygen therapy should strive to balance physiological efficacy with patient comfort.
