东莞地区献血者血小板基因型资料库容量分析OA
Analysis of the capacity of the blood platelet genotype database for blood donors in Dongguan region
目的 建立东莞市献血人群血小板人类白细胞抗原(HLA)、血小板特异性抗原(HPA)基因型资料库,估算适宜的库容量,为临床血小板输注治疗提供保障.方法 采用三代基因测序技术对 200 例纳入本站献血者血小板基因库建库标准的血样本进行HPA-1-15 和HLA-A、B 基因分型,统计分析HPA-1-15 和HLA-A、B 等位基因频率与单倍体型频率,依次推算至少找到 1 例 HLA、HPA 匹配献血者的概率与适宜的库容量.结果 本地区 HPA-1-15 和 HLA-A、B 具有多态性,根据单倍体型频率,估算当血小板献血者库容为 713 人时,单倍体型频率>0.0025 的患者有 95%的概率在基因库中至少找到 1 例 HPA-1-15 相匹配的献血者;建立 1 个库容为 2381 人的血小板献血者基因库可使单倍体型≥0.0025 的患者有 90%的可能找到至少 1 例 HLA-Ⅰ-A、B 相合的献血者,95%的概率则库容量为 4287 人,如果要同时满足与 HLA 和 HPA 完全匹配,库容量至少需 7721 人.结论 血小板献血者资料库容量估算直接影响建库的有效性,东莞市已经建立了 200 名献血者 HLA 和 HPA 基因型的资料库,但需继续扩大库容量.
Objective To establish a database of platelet human leukocyte antigen(HLA)and human platelet antigen(HPA)genotypes for blood donors in Dongguan City,and estimate the appropriate capacity of the database,so as to pro-vide a guarantee for clinical platelet transfusion therapy.Methods HPA-1-15 and HLA-A,B genotyping was performed on 200 blood samples included in the platelet gene bank construction standard of blood donors in this station by three-generation gene sequencing technology.The allele frequency and haplotype frequency of HPA-1-15 and HLA-A,B were statistically an-alyzed,and the probability of finding at least one HLA and HPA matching blood donor and the appropriate bank capacity were calculated in turn.Results HPA-1-15 and HLA-A,B were polymorphic in this region.According to the frequency of haplotypes,it was estimated that when the platelet donor pool capacity was 713 people,the patients with haplotype frequency>0.0025 had a 95%probability of finding at least one HPA-1-15 matched blood donor in the gene pool.The establishment of a platelet donor gene pool with a pool of 2381 people could make the patients with haplotype≥0.0025 had a 90%proba-bility of finding at least one HLA-Ⅰ-A,and B matched blood donor.If it was necessary to fully cooperate with HLA and HPA at the same time,the pool capacity must be at least 7721 people.Conclusion The estimation of the capacity of the platelet donor database directly affects the effectiveness of its establishment.Dongguan has already established a database of HLA and HPA genotypes for 200 donors,but it needs to continue expanding the capacity of the database.
