目的 构建基于血管生成拟态(VM)相关基因的肝细胞癌(HCC)预后模型.方法 从 TCGA 数据库获取HCC 患者数据,通过生存分析、受试者工作特征(ROC)曲线及校准曲线评估 ALYREF 与 HCC 预后的相关性;采用基因富集分析(GSEA)探究 ALYREF 与 VM 相关通路的关联.通过单因素 Cox 回归、最小绝对收缩和选择算子(LAS-SO)回归及随机森林(RF)算法筛选基于ALYREF 的VM 相关关键基因,构建ALYREF 相关VM 预后模型(AVMP)并计算风险评分(AVMP-Risk Score,AVMP-RS),单因素和多因素 Cox 回归分析 AVMP-RS 是否为独立预后因素,ROC、校准曲线及列线图评估 AVMP 的性能,最后以 ICGC 数据库中 HCC-LIHC 数据集作为外部验证集进行验证.结果 ALYREF 的表达水平在更高分期(Stage)和分级(Grade)的 HCC 患者中显著升高.High-ALYREF 组患者预后较差,且其表达水平对患者 1 年、2 年和 3 年生存率的预测具有较高的准确性.GSEA 分析显示,High-ALYREF 和 Low-ALYREF 组间的差异表达基因(DEGs)显著富集于TGF-B1、HIF1A、PI3K/AKT/mTOR、Notch、Wnt 和VEGFA/VEGFR2等经典 VM 通路.获得了 91 个 ALYREF 相关 VM 基因,LASSO 回归联合 RF 算法筛选出 KIF20A 和 CBX2 基因用于构建预后模型 AVMP.单因素和多因素 Cox 回归分析表明 AVMP-RS 是 HCC 患者预后的独立危险因素,其预测准确性优于其他临床因素(如年龄、性别、分期和分级),列线图可有效预测患者 3 年内的生存率.外部数据验证表明AVMP 具有较好的适用性.结论 研究成功构建了基于 ALYREF 的 VM 相关基因 HCC 预后模型 AVMP,在预测HCC 患者预后方面表现出较高的准确性,为识别高危患者和评估预后提供了重要参考.

Objective To construct a prognostic model for hepatocellular carcinoma(HCC)based on vasculogenic mim-icry(VM)related genes.Methods HCC patient data were retrieved from The Cancer Genome Atlas(TCGA)database.Correlation between ALYREF and HCC prognosis was assessed through survival analysis,receiver operating characteristic(ROC)curves,and calibration curves.Gene set enrichment analysis(GSEA)was employed to investigate association between ALYREF and VM related pathways.Univariate Cox regression,least absolute shrinkage and selection operator(LASSO)regression,and random forest(RF)algorithm were performed to screen ALYREF-based VM related key genes.An ALYREF-related vasculogenic mimicry prognostic model(AVMP)was constructed and the risk score(AVMP-Risk Score,AVMP-RS)was calculated.Univariate and multivariate Cox regression analysis were conducted to determine whether AVMP-RS was an independent prognostic factor.The performance of AVMP was evaluated using ROC curves,calibration curves,and nomograms.Finally,the HCC-LIHC dataset from the ICGC database was used as an external validation set for verification.Results The expression level of ALYREF was significantly elevated in HCC patients at higher stages(Stage)and grades(Grade).The prognosis of patients in the High-ALYREF group was poor,and its expression levels had high ac-curacy in predicting the 1-year,2-year,and 3-year survival rates of patients.GSEA analysis showed that DEGs between the High-ALYREF group and Low-ALYREF group were significantly enriched in classic VM pathways such as TGF-B1,HIF1A,PI3K/AKT/mTOR,Notch,Wnt,and VEGFA/VEGFR2.91 ALYREF related VM genes were obtained,and LASSO regres-sion combined with RF algorithm was used to screen KIF20A and CBX2 genes for constructing the prognostic model AVMP.Univariate and multivariate Cox regression analysis showed that AVMP-RS was an independent risk factor for the prognosis of HCC patients,and its predictive accuracy was superior to other clinical factors such as age,gender,staging,and grading.Column charts could effectively predict the survival rate of patients within 3 years.External data validation indicated that AVMP had good applicability.Conclusion This study successfully constructs a VM related gene HCC prognosis model AVMP based on ALYREF,which shows high accuracy in predicting the prognosis of HCC patients and provides important reference for identifying high-risk patients and evaluating prognosis.
