不同慢性病管理模式在2型糖尿病管理中的研究进展OA

With the increasing proportion of diabetes patients in China,traditional management models face a triple gap in diabe-tes"awareness-treatment-control".This article transcends the conventional geographical classification framework and,from three theo-retical dimensions—system reconstruction,self-management empowerment,and technology integration(encompassing both digital tech-nology and the synergy of traditional Chinese and Western medicine)—provides an in-depth deconstruction of the mechanisms of action and progress of common management models both at home and abroad.It aims to offer insights for exploring diabetes management mod-els better suited to China's national conditions and more efficient and comprehensive,further optimizing diabetes management strate-gies,and ultimately improving the overall health status and quality of life of diabetic patients.
