宏基因二代测序对不明原因宫内感染的病原学诊断价值OA
Value of Metagenomics Next-generation Sequencing in Etiological Diagnosis of Unexplained Intrauterine Infection
目的 探讨宏基因二代测序对不明原因宫内感染的诊断效能,以阐明其诊断价值和临床意义.方法 选取潍坊市妇幼保健院 2024 年 1 月-7 月收治的 2 例不明原因宫内感染患者(感染组)及 1 例对照病例(对照组),收集患者的临床信息,比较传统检测方法与宏基因组测序在阳性率及病原体分布上的差异.对样本进行DNA提取,经过质量控制后构建文库,并通过高通量测序获得数据.测序数据经Kraken2 软件物种注释后与UHGG和NCBI数据库对比,用于病原微生物的分类和鉴定,同时通过KEGG等进行生物信息学深度分析.结果 宏基因组测序在病原体检测阳性率、细菌感染检测阳性率均高于传统检测方法(P<0.05),而在衣原体、病毒、支原体和真菌感染检测中未见差异(P>0.05);感染组病原体种类少于对照组,且感染组存在 10 种独有病原体(如结核棒状杆菌、詹氏乳杆菌);富集分析发现基因主要集中在核糖体相关通路,GSEA分 析 显 示,MRPS12 和MRPL23 两个关键分子主要在视黄醇代谢、细胞色素代谢及免疫相关通路中发挥重要作用.结论 相较于传统检测,宏基因检测在不明原因宫内感染的患者有更好的诊断性能,尤其在罕见或难培养病原体上.
Objective To investigate the diagnostic efficacy of metagenomic next-generation sequencing for unexplained intrauterine infection,and to clarify its diagnostic value and clinical significance.Methods Two patients with unexplained intrauterine infection(infection group)and one control case(control group)admitted to Weifang Maternal and Child Health Hospital from January to July 2024 were selected.The clinical information of the patients was collected,and the differences in positive rate and pathogen distribution between traditional detection methods and metagenomic sequencing were compared.After DNA extraction of the samples,the library was constructed after quality control,and the data were obtained by high-throughput sequencing.The sequencing data were annotated by Kraken2 software and compared with UHGG and NCBI database for the classification and identification of pathogenic microorganisms,and the bioinformatics depth analysis was carried out by KEGG.Results The positive rate of metagenomic sequencing in pathogen detection and bacterial infection detection was higher than that of traditional detection methods(P<0.05),but there was no difference in the detection of chlamydia,virus,mycoplasma and fungal infection(P>0.05).The types of pathogens in the infection group were less than those in the control group,and there were 10 unique pathogens in the infection group(such as Corynebacterium tuberculosis,Lactobacillus johnsonii).Enrichment analysis showed that the genes were mainly concentrated in ribosome-related pathways.GSEA analysis showed that MRPS12 and MRPL23 played an important role in retinol metabolism,cytochrome metabolism and immune-related pathways.Conclusion Compared with traditional detection,metagenomic detection has better diagnostic performance in patients with unexplained intrauterine infection,especially in rare or difficult to cultivate pathogens.
