Effect of electroacupuncture combined with modified Decoction for dissipating bold stasis under diaphragm on chronic salpingitis with syndrome of Qi stagnation and blood stasis in rats

目的 探讨电针联合膈下逐瘀汤加减方对气滞血瘀型慢性输卵管炎大鼠的影响及其作用机制.方法 将48 只雌性 SD 大鼠随机分为空白组、模型组、膈下逐瘀汤组、电针联合膈下逐瘀汤组,每组12 只.空白组大鼠不做处置,其余组大鼠均制备气滞血瘀型慢性输卵管炎模型.实验第29 天,模型组给予纯净水灌胃,膈下逐瘀汤组给予 1.50 g/mL 膈下逐瘀汤加减方混悬液灌胃,电针联合膈下逐瘀汤组给予1.50 g/mL 膈下逐瘀汤加减方混悬液灌胃和电针干预,均1 次/d,干预4 周.比较各组大鼠气滞血瘀证候积分,HE 染色观察输卵管组织病理形态,ELISA 法检测输卵管组织中Ⅰ型胶原蛋白(Col-Ⅰ)、基质金属蛋白酶抑制剂-1(TIMP-1)、基质金属蛋白酶-9(MMP-9)含量及 MMP-9/TIMP-1 比值,免疫组化法检测输卵管组织中 MMP-9、TIMP-1 阳性表达情况,RT-qPCR 法检测血清中 Col-Ⅰ mRNA 表达情况.结果 与空白组比较,模型组大鼠气滞血瘀证总积分明显升高(P<0.05);输卵管炎性损伤明显;输卵管组织中Col-Ⅰ含量、TIMP-1 含量、TIMP-1 阳性表达平均光密度值及血清 Col-Ⅰ mRNA 相对表达量均明显升高(P 均<0.05),MMP-9 含量、MMP-9/TIMP-1 比值及MMP-9 阳性表达平均光密度值均明显降低(P 均<0.05).与模型组比较,膈下逐瘀汤组、电针联合膈下逐瘀汤组大鼠气滞血瘀证总积分明显降低(P均<0.05);输卵管炎性损伤明显减轻;输卵管组织中 Col-Ⅰ含量、TIMP-1 含量、TIMP-1 阳性表达平均光密度值及血清 Col-Ⅰ mRNA 相对表达量均明显降低(P 均<0.05),MMP-9 含量、MMP-9/TIMP-1 比值及 MMP-9 阳性表达平均光密度值均明显升高(P 均<0.05);与膈下逐瘀汤组比较,电针联合膈下逐瘀汤组上述各指标改善更为明显(P 均<0.05),输卵管炎性损伤更轻.结论 电针联合膈下逐瘀汤加减方可明显减轻气滞血瘀型慢性输卵管炎大鼠输卵管组织炎性损伤和纤维化程度,机制可能与抑制胶原沉积,调节MMP-9与TIMP-1动态平衡,干预细胞外基质代谢有关.

Objective It is to investigate the effect and mechanism of electroacupuncture(EA)combined with modified Decoction for dissipating bold stasis under diaphragm(DDBSUD)on chronic salpingitis with syndrome of Qi stagnation and blood stasis in rats.Methods Forty-eight female SD rats were randomly divided into normal group,model group,DDBSUD group,EA+DDBSUD group,with 12 rats in each group.The rats in the control group were not treated,while the rats in the other groups were prepared to establish models of chronic salpingitis with syndrome of Qi stagnation and blood stasis.On the 29th day of the experiment,the model group was administered purified water by gavage,the DDBSUD group was adminis-tered 1.50 g/mL suspension of DDBSUD by gavage,and the EA+DDBSUD group were treated with both EA and 1.50 g/mL suspension of DDBSUD by gavage,all administered once daily,continuously treated for 4 weeks.The scores of syndrome of Qi stagnation and blood stasis of the rats were compared among the groups,the histopathological morphology of fallopian tube tissue was observed by HE staining,the contents of type Ⅰ collagen(Col-Ⅰ),tissue inhibitor of metalloproteinases-1(TIMP-1),matrix metalloproteinase-9(MMP-9)and the MMP-9/TIMP-1 ratio in fallopian tube tissues were measured by ELISA,the positive expressions of MMP-9 and TIMP-1 in fallopian tube tissues were detected by immunohistochemistry,and the expression of Col-Ⅰ mRNA in serum was detected by RT-qPCR.Results Compared with the control group,the to-tal score of syndrome of Qi stagnation and blood stasis of rats in the model group was significantly increased(P<0.05);significant inflammatory damage was observed in the fallopian tubes;the contents of Col-Ⅰ and TIMP-1,the mean optical density of TIMP-1 positive expression in fallopian tube tissue,and the relative expression of Col-Ⅰ mRNA in serum were all significantly increased(all P<0.05),while the content of MMP-9,MMP-9/TIMP-1 ratio,and the mean optical densi-ty of MMP-9 positive expression were all significantly reduced(all P<0.05).Compared with the model group,the total scores of syndrome of Qi stagnation and blood stasis of rats in the DDBSUD group and EA+DDBSUD group were signifi-cantly decreased(both P<0.05);the inflammatory damages in the fallopian tubes were alleviated;the contents of Col-Ⅰand TIMP-1,the mean optical density of TIMP-1 positive expression in fallopian tube tissue,and the relative expression of Col-Ⅰ mRNA in serum were all significantly decreased(all P<0.05),while the contents ofMMP-9,MMP-9/TIMP-1 ra-tios,and the mean optical densities of MMP-9 positive expression were all significantly increased(all P<0.05),and the improvements of these indicators and inflammatory damages in the fallopian tubes in the EA+DDBSUD group were more significant than those in the DDBSUD group(all P<0.05).Conclusion EA combined with DDBSUD can significantly al-leviate inflammatory damage and fibrosis in the fallopian tubes of rats with chronic salpingitis with syndrome of Qi stagnation and blood stasis;the mechanism may be related to inhibition of collagen deposition,regulation of the dynamic balance be-tween MMP-9 and TIMP-1,and the regulation of extracellular matrix metabolism.
