基于因子分析与聚类分析的帕金森病患者中医证候特征及温肾养肝方对其血清炎症因子和神经营养因子水平的影响OA
TCM Syndrome Characteristics of Parkinson's Disease Patients and the Effects of Wenshen Yanggan Formula on Their Serum Inflammatory and Neurotrophic Factor Levels Based on Factor Analysis and Clustering Analysis
目的 基于因子分析与聚类分析探讨帕金森病患者中医证候特征及温肾养肝方对其血清炎症因子和神经营养因子水平的影响.方法 选取2021年12月—2023年11月广西中医药大学第一附属医院收治的80 例帕金森病患者作为研究对象,进行回顾性分析.根据是否选用温肾养肝方治疗,将患者分为对照组和观察组,每组40 例.采用KMO检验和Bartlett球形检验分析数据是否符合因子分析前提条件,对收集的帕金森病症状进行因子分析与聚类分析.采用重复测量方差分析评估温肾养肝方治疗帕金森的疗效.结果 帕金森病常见症状有震颤、运动迟缓等.共提取8个公因子,涉及内风、阴虚等证候要素,经聚类分析,聚为3类时症状辨证清晰且贴合临床,即内热阳亢、脾阳亏虚、气血两虚.治疗后两组患者中医证候积分较治疗前降低,差异有统计学意义(P<0.05);且观察组中医证候积分低于对照组,差异有统计学意义(P<0.05).治疗后3种聚类患者中医证候积分均显著下降,且每类积分下降差值差异显著,其中第3类气血两虚型证候积分下降幅度最大,差异有统计学意义(P<0.05).主效应分析结果显示,血清炎症因子和神经营养因子指标的时间效应、组间效应均有统计学意义(P<0.001);两组患者白细胞介素-6(Interleukin-6,IL-6)、肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)、脑源性神经营养因子(Brain-derived neurotrophic factor,BDNF)、胶质源性神经营养因子(Glial-derived neurotrophic factor,GDNF)水平交互效应,差异有统计学意义(P<0.05).治疗后两组患者IL-6、TNF-α水平较治疗前降低,GDNF、BDNF水平较治疗前升高,差异有统计学意义(P<0.05);且观察组IL-6、TNF-α水平低于对照组,GDNF、BDNF水平高于对照组,差异有统计学意义(P<0.05).结论 将选取的34种症状聚为3类,分别为内热阳亢、脾阳亏虚、气血两虚.采用温肾养肝方治疗帕金森病患者后,中医证候积分明显下降,其中下降最明显的是气血两虚型患者;同时,温肾养肝方能够有效降低帕金森患者的血清炎症水平,促进神经营养因子的释放.
Objective To explore the traditional Chinese medicine(TCM)syndrome characteristics of Parkinson's disease(PD)patients and the effects of Wenshen Yanggan Formula on their serum inflammatory and neurotrophic factor levels based on factor analysis and clustering analysis.Methods A retrospective analysis was conducted on 80 PD patients admitted to the First Affiliated Hospital of Guangxi University of Chinese Medicine from December 2021 to November 2023.The patients were divided into a control group and an observation group based on whether Wenshen Yanggan Decoction was included in the treatment,with 40 patients in each group.Then,Kaiser-Meyer-Olkin(KMO)and Bartlett's Test of Sphericity were employed to assess whether the data fulfilled the prerequisites for factor analysis before applying factor and clustering analyses to the PD symptoms collected.Finally,a repeated measure analysis of variance(ANOVA)was conducted to evaluate the efficacy of Wenshen Yanggan Decoction on PD.Results Common symptoms of PD included tremor and bradykinesia.Eight common factors were extracted,involving syndrome elements such as internal wind and yin deficiency.Through clustering analysis,the symptoms were classified into three clusters,namely internal heat and yang hyperactivity,spleen yang deficiency,as well as qi and blood deficiency.The clustering results were clear and consistent with clinical practice.After treatment,the TCM syndrome scores were decreased in both groups(P<0.05),and the scores were significantly lower in the observation group(P<0.05).According to clustering analysis,the TCM syndrome scores were significantly decreased in the patients of the three clusters,with statistically significant differences in the score reductions.Among these clusters,the greatest score reduction was observed in the third cluster,qi and blood deficiency type(P<0.05).The main effect analysis results revealed statistical significance in the time effect and the group effect of serum inflammatory factors and neurotrophic factors(P<0.001),as well as in the interaction effects of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),brain-derived neurotrophic factor(BDNF),and glial-derived neurotrophic factor(GDNF)levels between the two groups(P<0.05).IL-6 and TNF-α levels were decreased,while GDNF and BDNF levels were increased in both groups(P<0.05),and the differences were more significant in the observation group(P<0.05).Conclusion In this study,34 selected symptoms were clustered into 3 categories:internal heat and yang hyperactivity,spleen yang deficiency,along with qi and blood deficiency.After treatment with Wenshen Yanggan Decoction,TCM syndrome scores were decreased significantly,especially for patients with qi and blood deficiency.Additionally,the Decoction effectively lowers serum inflammation levels and promotes neurotrophic factor release.
