目的 探究补肾柔肝汤辅助治疗卵巢储备功能下降(Decreased ovarian reserve,DOR)肾虚肝郁型患者的临床疗效及对其炎症、卵巢微环境的影响.方法 选取2022年1月—2024年1月于中国中医科学院眼科医院就诊的87例DOR患者,按随机数字表法分为对照组44 例和观察组43 例.对照组予以芬吗通治疗,观察组在对照组治疗的基础上联合补肾柔肝汤治疗.治疗3个月经周期后,观察比较两组患者临床疗效、不良反应情况,治疗前后中医证候积分、实验室指标[血清卵泡刺激素(Follicle stimulating hormone,FSH)、黄体生成素(Luteinizing hormone,LH)、抗缪勒管激素(Anti-müllerian hormone,AMH)、血清生长分化因子-9(Growth differentiation factor 9,GDF-9)、Toll样受体2(Toll-like receptor 2,TLR2)]及卵巢微环境状态[卵巢动脉收缩期峰值流速(Peak systolic velocity,PSV)].结果 治疗后观察组临床总有效率95.35%(41/43)高于对照组79.55%(35/44),差异有统计学意义(P<0.05).治疗后两组患者中医证候积分均较治疗前降低,差异有统计学意义(P<0.05);且观察组中医证候积分明显低于对照组,差异有统计学意义(P<0.05).治疗后两组患者FSH、LH水平均较治疗前降低,AMH水平均较治疗前升高,差异有统计学意义(P<0.05);且观察组FSH、LH水平均明显低于对照组,AMH水平明显高于对照组,差异有统计学意义(P<0.05).治疗后两组患者TLR2水平均较治疗前降低,PSV、血清GDF-9水平均较治疗前升高,差异有统计学意义(P<0.05);且观察组TLR2水平明显低于对照组,PSV、血清GDF-9水平均明显高于对照组,差异有统计学意义(P<0.05).治疗期间,两组患者不良反应发生率比较,差异无统计学意义(P>0.05).结论 补肾柔肝汤辅助治疗DOR肾虚肝郁型可提高疗效,减轻月经异常症状,调节性激素水平,抑制炎症,改善卵巢功能及卵巢微环境状态.

Objective To investigate the clinical efficacy of Bushen Rougan Decoction as an adjunctive therapy for diminished ovarian reserve(DOR)presenting with the kidney deficiency and liver depression pattern,and to examine its effects on inflammation and ovarian microenvironment.Methods Eighty-seven DOR patients treated at the Eye Hospital of China Academy of Chinese Medical Sciences from January 2022 to January 2024 were enrolled and randomly divided into a control group(n=44)and an observation group(n=43)using a random number table.The control group received Femoston treatment,while the observation group received Bushen Rougan Decoction combined with Femoston.After three menstrual cycles of treatment,the clinical efficacy and adverse reactions were compared between the two groups.The assessments included the traditional Chinese medicine(TCM)syndrome scores,laboratory indicators[serum follicle-stimulating hormone(FSH),luteinizing hormone(LH),anti-Müllerian hormone(AMH),growth differentiation factor-9(GDF-9),and Toll-like receptor 2(TLR2)],and ovarian microenvironment status[peak systolic velocity(PSV)of ovarian arteries]measured before and after treatment.Results The total clinical effective rate in the observation group was 95.35%(41/43),which was significantly higher than that in the control group(79.55%,35/44)(P<0.05).After treatment,the TCM syndrome scores in both groups were significantly lower than those before treatment(P<0.05),with the observation group showing significantly lower scores than the control group(P<0.05).After treatment,the levels of FSH and LH in both groups were significantly lower than before treatment,while the AMH levels were significantly higher(P<0.05).Moreover,the FSH and LH levels in the observation group were significantly lower than those in the control group,while the AMH level was significantly higher(P<0.05).After treatment,the TLR2 levels significantly decreased in both groups,while the PSV and serum GDF-9 levels significantly increased in both groups(P<0.05).Additionally,the observation group showed significantly lower TLR2 level and significantly higher PSV and serum GDF-9 level than the control group(P<0.05).No statistically significant difference was observed in the incidence of adverse reactions between the two groups during the treatment(P>0.05).Conclusion As an adjunctive therapy,Bushen Rougan Decoction can improve clinical efficacy,alleviate menstrual abnormalities,regulate sex hormone levels,suppress inflammation,and improve ovarian function and the ovarian microenvironment in DOR patients with the kidney deficiency and liver depression pattern.
