关节镜下"T"形入路松解治疗臀肌挛缩症的临床研究OA

Objective To investigate the surgical technique and clinical efficacy of arthroscopic release via T-shaped portals for treatment of gluteal muscle contracture(GMC).Methods Clinical data from 60 patients(120 hips)with GMC treated arthroscopically through the posterosuperior trochanteric portal and the posteroinferior trochanteric portal at Foshan Hospital of Traditional Chinese Medicine between December 2021 and June 2024 were retrospectively analyzed.All patients underwent standardized postoperative early functional rehabilitation.Hip function,complications,and subjective patient satisfaction were evaluated at follow-up.Results Patients were followed up for 10 to 40 months,with an average of(22.6±6.4)months.The mean operative time per hip was(21.7±8.8)min,and the mean length of hospital stay was(4.7±1.2)d.No severe complications,such as infections or neurovascular injuries,occurred postoperatively.At final follow-up,the median GMC functional score significantly improved from preoperative 52.0 points(45.0,60.0)to postoperative 88.0 points(81.0,93.0)(P<0.05).Significant improvements were observed in hip adduction and internal rotation,with positive signs such as the Cross-leg test and Ober's test notably decreased.Subjective patient satisfaction reached 95.0％.Conclusion Arthroscopic release via T-shaped portals is associated with stable surgical manipulation,clear visualization,comprehensive contracture release,minimal trauma,and low complication rates.Combined with standardized postoperative rehabilitation,this method significantly improves hip function and quality of life,showing favorable potential for clinical application.
