Return-to-work experience of young and middle-aged breast cancer survivors:a qualitative study

目的 探讨中青年乳腺癌生存者重返工作体验,为制定乳腺癌患者职业康复支持方案提供依据.方法 基于罗伊适应理论,通过描述性质性研究方法,目的抽样法选取2025年7-10月桂林市某三级甲等医院乳腺外科复诊、已重返工作的14名中青年乳腺癌生存者进行半结构式访谈,采用定向内容分析法进行资料分析、主题提炼.结果 共提炼出4个主题和12个亚主题:职业能力受损(治疗副作用影响工作表现、认知功能下降限制工作效率、体能衰退制约工作强度);心理适应障碍(身体意象紊乱降低工作自信、复发恐惧干扰职业规划、病耻感阻碍职场融入);社会支持需求(家庭支持缓解复工压力、同事理解促进职场适应、医疗指导保障工作安全);制度环境制约(制度性障碍增加复工阻力、保险缺失加剧经济压力、弹性工作制面临诸多挑战).结论 中青年乳腺癌生存者重返工作面临身体机能、心理适应、社会支持和制度环境的多重挑战,建议医疗机构联合用人单位构建包含职业评估、心理支持、职场适应训练和制度保障的综合康复体系,促进乳腺癌生存者社会功能的全面恢复.

Objective To explore the experience of return to work of young and middle-aged breast cancer survivors,and to provide evidence for the development of vocational rehabilitation support programs for this group of population.Methods Based on Roy Adaptation Model,a descriptive qualitative study was conducted.A purposive sampling was used to select 14 young and middle-aged breast cancer survivors who were follow-up patients at the breast surgery department of a tertiary grade-A hospital in Guilin and had returned to work between July and October 2025.Semi-structured interviews were conducted,and data were analyzed using directed content analysis to extract themes.Results Four themes and twelve sub-themes were identified:impaired occupational capacity(treatment side effects affecting work performance,decline in cognitive function limiting work efficiency,and physical decline con-straining work intensity);psychological adaptation disorders(body image disturbance reducing work confidence,fear of recurrence disrupting career planning,and stigma hindering workplace integration);social support needs(family support alleviating return to work stress,colleague understanding facilitating workplace adaptation,and medical guidance ensuring work safety);and institutional environ-mental constraints(institutional barriers increasing return-to-work barriers,lack of insurance exacerbating financial pressure,and flexible work arrangements facing multiple challenges).Conclusion Young and middle-aged breast cancer survivors face multiple challenges when returning to work,including physical dysfunction,psychological adaptation,social support,and institutional environ-ment.It is recommended that medical institutions collaborate with employers to establish a comprehensive rehabilitation system encom-passing occupational assessment,psychological support,workplace adaptation training,and institutional safeguards,thereby promoting the full recovery of social function in breast cancer survivors.
