针对城中村中老年慢性病患者自我管理依从性不足的问题,本研究整合计划行为理论与HOOKED模型,构建扩展行为理论框架.以广州市城中村中老年慢性病患者为对象,综合运用半结构化访谈、问卷调查与结构方程模型,识别影响其自我管理行为的关键因素.基于201份有效问卷的分析显示:行为态度、主观规范、感知行为控制与酬赏均显著正向影响行为意图;投入虽无直接影响,但通过增强认知中介产生间接效应.据此,提出"触发—酬赏—投入"三位一体设计策略,并开发"康伴"APP进行实践验证.可用性测试SUS评分为85.83分,处于"优秀"等级,证实该策略能有效跨越数字鸿沟并提升管理依从性.本研究为特殊场景下的适老化健康管理产品设计提供了理论依据与实践路径.

Aiming at the problem of insufficient self-management compliance of middle-aged and elderly patients with chronic diseases in urban villages,this study integrates the theory of planned behavior and the HOOKED model to construct an extended behavior theory framework.Taking the middle-aged and elderly patients with chronic diseases in urban villages in Guangzhou as the object,semi-structured interviews,questionnaires and structural equation model were used to identify the key factors affecting their self-management behavior.Based on the analysis of 201 valid questionnaires,it is found that behavioral attitude,subjective norm,perceived behavioral control and reward all have significant positive effects on behavioral intention.Although input has no direct impact,it has an indirect effect by enhancing cognitive mediation.Based on this,the'trigger-reward-input'trinity design strategy is proposed,and the'Kangban'APP is developed for practical verification.The SUS score of the usability test was 85.83,which was in the'excellent'level,confirming that the strategy can effectively cross the digital divide and improve management compliance.This study provides a theoretical basis and practical path for the design of aging health management products in special scenarios.
