专业领域内部相关利益者视角下营养支持专科护士角色期望的质性研究OA
Role expectations for nutrition support nurse from the perspective of intra-professional stakeholders:a qualitative study
目的 以角色理论框架为基础,探讨专业领域内部相关利益者视角下营养支持专科护士的角色期望,了解营养支持专科护士的角色内涵,为营养支持专科护士的临床使用提供依据.方法 以角色理论为框架,采用目的抽样法于2025 年 1 月选取2020-2024年在武汉市某三级甲等医院培训基地参加中华护理学会营养支持专科护士培训并已取得证书的护理人员,以及相关专业领域内部利益相关者包括临床营养师、护理管理者、临床医师为访谈对象,进行半结构式访谈,采用定向内容分析法对访谈资料进行整理、分析.结果 最终共纳入19名访谈对象,对营养支持专科护士角色期望归纳为4个主题,即素质期望(具备专业素养、具备教学培训能力、具备预见性思维)、形象期望(营养护理的专业标杆、分工明确的协作者、创新实践的探索者)、义务期望(营养护理实践者、质量控制监督者、健康教育推广者)和行为期望(专业自主性与规范性、持续学习与专业深耕).结论 本研究构建了包含素质、形象、义务、行为的四维营养支持专科护士角色期望框架,描绘出营养支持专科护士复合型专家形象的同时也揭示了其内在的张力与冲突,建议通过制度化职责划分、提升营养支持专科护士决策权限,明确分工与职能等方式推动其临床实践能力的转化,为护理专科化发展及护理服务质量的持续改进提供参考.
Objective To explore the role expectations for nutrition support nurse(NSN)from the perspective of intra-professional stakeholders based on role theory,to understand the role connotation of NSN,and to provide reference for clarifying their clinical roles.Methods Guided by role theory,purposive sampling was employed in January 2025 to select participants who completed the Chinese Nursing Association's NSN clinical practice training at a tertiary grade-A hospital in Wuhan from 2020 to 2024,along with key stake-holders including clinical nutritionists,nursing managers,and physicians.Semi-structured interviews were conducted,and the data were analyzed using directed qualitative content analysis.Results Nineteen participants were included.The role expectations for NSNs were summarized into four themes:competency expectation(professional expertise,teaching and training ability,and anticipatory thinking),image expectation(serving as a professional benchmark in nutrition care,a clear collaborator,and an explorer of innovative practices),obligation expectation(acting as a practitioner of nutrition care,a supervisor of quality control,and a promoter of health education),and behavioral expectation(balancing professional autonomy with standardization,and committing to continuous learning and professional in-depth research).Conclusion This study constructs a four-dimensional framework of role expectations for NSNs,encompassing competency,professional image,obligation,and behavior expectation.While delineating a composite expert profile for these nurses,the framework also reveals the inherent tensions and conflicts embedded within these role expectations.To facilitate the translation of their clinical competencies into practice,it is recommended to implement measures such as institutionalized role delinea-tion,enhanced decision-making authority,and clarified interdisciplinary divisions and functions,thus to provide reference for advanc-ing nursing specialization and the continuous improvement of nursing care quality.
