基于非门控胸部CT的冠状动脉钙化与冠状动脉狭窄程度相关性OA
Association of coronary artery calcification on non-gated chest CT with the severity of coronary artery stenosis
目的 探讨基于非门控胸部CT的冠状动脉钙化(CAC)与冠心病(CHD)及冠状动脉狭窄程度的相关性.方法 回顾性纳入2022年1-12月在南京医科大学附属淮安第一医院行冠状动脉造影及胸部CT检查的2 105例疑似CHD病人.依据Agatston评分将病人分为无钙化及轻、中、重度钙化组.将冠状动脉狭窄≤50%者纳入对照组(528例),>50%者纳入CHD组(1 577例).比较两组基线资料及CAC分布,采用多因素Logistic回归分析CAC与CHD的关系,采用卡方检验分析CAC与狭窄程度的相关性.结果 CHD组年龄、男性比例、吸烟率、糖尿病占比以及甘油三酯水平均高于对照组(x2=15.935~34.498,Z=-4.925、-2.863,P<0.05).CAC程度与CHD存在显著关联(x2=297.480,P<0.001).随着钙化程度加重,冠状动脉狭窄程度显著增加(x2=437.641,P<0.001).CAC是CHD的独立危险因素,轻、中、重度钙化组发生CHD的风险分别是无钙化组的3.39、11.46、27.39倍(P<0.001).结论 基于非门控胸部CT的CAC与CHD及冠状动脉狭窄程度显著相关,可作为CHD诊断和严重程度评估的参考依据.
Objective To investigate the association of coronary artery calcification(CAC)assessed by non-gated chest CT with coronary heart disease(CHD)and the severity of coronary artery stenosis.Methods A retrospective analysis was per-formed for 2 105 patients suspected of CHD who underwent both invasive coronary angiography and chest CT examination in Huai'an No.1 People's Hospital Affiliated to Nanjing Medical University from January to December 2022.According to the Agat-ston score,the patients were divided into non-calcification group,mild calcification group,moderate calcification group,and severe calcification group.The 528 patients with coronary artery stenosis ≤50%were included in the control group,while the 1 577 pa-tients with coronary artery stenosis>50%were included in the CHD group.Baseline data and CAC distribution were compared be-tween the two groups.The multivariate logistic regression analysis was used to investigate the association between CAC and CHD,and the chi-square test was used to analyze the correlation between CAC and the severity of stenosis.Results Compared with the control group,the CHD group had significantly higher age,proportion of male patients,smoking rate,prevalence rate of dia-betes,and level of triglyceride(x2=15.935-34.498,Z=—4.925,—2.863,P<0.05).The degree of CAC was significantly associ-ated with CHD(x2=297.480,P<0.001).The severity of coronary artery stenosis increased significantly with the increase in the degree of calcification(x2=437.641,P<0.001).CAC was an independent risk factor for CHD,and the risk of developing CHD in the mild,moderate,and severe calcification groups was 3.39,11.46,and 27.39 times,respectively,that in the non-calcification group(P<0.001).Conclusion CAC assessed by non-gated chest CT is significantly associated with CHD and the severity of coronary artery stenosis,which provides a reference for the diagnosis of CHD and the assessment of disease severity.
