神经梅毒血脑屏障破坏机制与诊断标志物的研究进展OA
Research progress on blood-brain barrier disruption mechanisms and diagnostic biomarkers in neurosyphilis
梅毒螺旋体能够侵入中枢神经系统,从而引发神经梅毒.神经梅毒的发病机制与梅毒螺旋体突破血脑屏障息息相关.神经梅毒的临床表现多样且常缺乏特异性,给早期诊断带来挑战.国内外神经梅毒诊断指南均提示神经梅毒的诊断需要结合流行病学史、临床表现及实验室检测结果.本文系统梳理近年国内外关于神经梅毒的研究文献,分析梅毒螺旋体突破血脑屏障的细胞与分子机制,并总结脑脊液差异代谢物、趋化因子、蛋白质及血清标志物在神经梅毒的诊断中的价值,为神经梅毒诊断提供参考.
Treponema pallidum is capable of invading the central nervous system,thereby cau-sing neurosyphilis.The pathogenesis of neurosyphilis is closely related to the ability of T.pallidum to penetrate the blood-brain barrier.The clinical manifestations of neurosyphilis are diverse and of-ten lack specificity,which poses challenges for early diagnosis.Both domestic and international guidelines for the diagnosis of neurosyphilis indicate that diagnosis should be based on a compre-hensive assessment of epidemiological history,clinical manifestations,and laboratory test results.This article systematically reviews recent domestic and international literature on neurosyphilis,an-alyzes the cellular and molecular mechanisms underlying blood-brain barrier disruption by T.palli-dum,and summarizes the diagnostic value of differential cerebrospinal fluid metabolites,chemo-kines,proteins,and serum biomarkers,with the aim of providing references for the diagnosis of neurosyphilis.
