Clinical efficacy of femoral neck system internal fixation versus cannulated screw internal fixation in the treatment of femoral neck frac-tures

目的 比较股骨颈动力交叉钉系统(FNS)与空心钉内固定术治疗股骨颈骨折的临床效果.方法 选取2020年6月—2023年6月间因股骨颈骨折住院的86例病人为研究对象,随机分为观察组和对照组,每组43例病人.对照组接受空心钉内固定术治疗,观察组接受FNS内固定术治疗,比较两组手术相关指标、临床疗效及并发症等指标.结果 观察组病人术中出血量显著高于对照组(t=17.055,P<0.05),手术时间、透视次数、住院时间、下地活动时间显著低于对照组(t=3.108~15.788,P<0.05).观察组术后3个月的临床疗效、Harris评分显著高于对照组(Z=8.427,t=3.123,P<0.05),术后负重时间、骨折愈合时间、股骨颈缩短长度均显著低于对照组(t=3.438~15.959,P<0.05).术后3个月时,观察组Ⅰ型胶原交联C端肽水平低于对照组,骨钙素水平高于对照组(t=9.731、2.707,P<0.05).观察组术后并发症发生率低于对照组(x2=8.795,P<0.05).结论 采用FNS内固定术治疗股骨颈骨折具有更显著的临床效果,且安全性较高.

Objective To investigate the clinical efficacy of femoral neck system(FNS)internal fixation versus cannulated screw internal fixation in the treatment of femoral neck fractures.Methods A total of 86 patients with femoral neck fractures who were hospitalized from June 2020 to June 2023 were enrolled as subjects and were randomly divided into observation group and control group,with 43 patients in each group.The patients in the control group underwent cannulated screw internal fixation,while those in the observation group received FNS internal fixation,and the two groups were compared in terms of surgical met-rics,clinical outcome,and complications.Results Compared with the control group,the observation group had a significantly higher intraoperative blood loss(t=17.055,P<0.05),a significantly lower number of fluoroscopy sessions,and significantly shor-ter time of operation,length of hospital stay,and time for ambulation(t=3.108-15.788,P<0.05).Compared with the control group,the observation group had significantly better clinical outcome and Harris score at 3 months after surgery(Z=8.427,t=3.123,P<0.05),significantly shorter weight-bearing time and time to fracture healing,and significantly less femoral neck shorte-ning(t=3.438-15.959,P<0.05).At 3 months after surgery,compared with the control group,the observation group had a sig-nificantly lower level of cross-linked C-telopeptide of type Ⅰ collagen and a significantly higher level of osteocalcin(t=9.731,2.707,P<0.05).In addition,the observation group had a significantly lower incidence rate of complications than the control group(x2=8.795,P<0.05).Conclusion FNS internal fixation has a better clinical efficacy in the treatment of femoral neck frac-tures,with a favorable safety profile.
