目的 探讨长链非编码RNA(lncRNA)肺腺癌相关转录本1(MALAT1)在动脉瘤性蛛网膜下腔出血(aSAH)患者血清中的表达水平与病情严重程度及预后的相关性.方法 选择 2019 年 6 月至 2022 年 3月在本院进行治疗的118 例aSAH患者作为观察对象(aSAH组),其中Hunt-Hess分级Ⅰ~Ⅱ级67 例,Ⅲ~Ⅳ级51 例;根据术后6 个月预后情况分为预后良好亚组(65 例),预后不良亚组(53 例).另选取同期120 例体检人员作为对照组.用qRT-PCR法检测入组者血清lncRNA MALAT1 表达水平.使用Logistic回归模型分析影响aSAH患者预后不良的危险因素.结果 aSAH组高血压、糖尿病患者比率显著高于对照组(均P<0.05);aSAH组患者血清lncRNA MALAT1 表达水平(2.13±0.36)明显高于对照组(1.05±0.15),差异有统计学意义(t=30.298,P<0.05).Hunt-Hess分级Ⅲ~Ⅳ级aSAH患者血清lncRNA MALAT1 表达水平(2.41±0.32)明显高于Ⅰ~Ⅱ级aSAH患者(1.92±0.28),差异有统计学意义(t=8.851,P<0.05).预后不良亚组高血压病、Hunt-Hess分级Ⅲ~Ⅳ级、mFisher评分 3~4 分、脑血管痉挛、脑出血的比率及血清lncRNA MALAT1 的表达水平均明显高于预后良好亚组(均P<0.05).Logistic多因素分析结果表明,Hunt-Hess Ⅲ~Ⅳ级、mFisher评分3~4 分、脑血管痉挛、脑出血及血清lncRNA MALAT1 表达水平升高是aSAH患者预后不良的危险因素(均P<0.05).血清lncRNA MALAT1 诊断aSAH患者预后不良的ROC曲线下面积为 0.833(95%CI:0.753~0.895),最佳截断值为2.03,敏感度为 84.91%,特异度为 75.38%.结论 aSAH患者血清中lncRNA MALAT1 的表达显著上调,其表达水平与患者病情严重程度及预后密切相关,有望成为评估aSAH预后的辅助生物标志物.

Objective To investigate the correlation between the expression level of long non-coding RNA(lncRNA)metastasis-associated lung adenocarcinoma transcript 1(MALAT1)in serum of patients with aneurysmal subarachnoid hemorrhage(aSAH)and the severity and prognosis of the disease.Methods A total of 118 aSAH patients who were treated in our hospital from June 2019 to March 2022 were regarded as observation objects(aSAH group).Among them,there were 67 cases of Hunt-Hess gradeⅠ-Ⅱ and 51 cases of gradeⅢ-Ⅳ.According to postoperative prognosis in 6 months,the patients were categorized into good prognosis subgroup(65 cases)and poor prognosis subgroup(53 cases).In addition,120 medical examiners during the same period were regarded as the control group.QRT-PCR method was applied to detect the expression level of serum lncRNA MALAT1.Logistic regression model was applied to analyze the risk factors for poor prognosis in patients with aSAH.Results The incidence rates of hypertension and diabetes were substantially higher in the aSAH group than those in the control group(all P<0.05).The expression level of serum lncRNA MALAT 1 in the aSAH group(2.13±0.36)was significantly higher than that in the control group(1.05±0.15)(t=30.298,P<0.05).The expression level of serum lncRNA MALAT1 in patients with grade Ⅲ-Ⅳ aSAH(2.41±0.32)was significantly higher than that in patients with grade Ⅰ-Ⅱ aSAH(1.92±0.28)(t=8.851,P<0.05).The percentage of patients with hypertension,the rate of Hunt-Hess grade Ⅲ-Ⅳ,mFisher score 3-4,the proportions of patients with cerebral vasospasm and cerebral hemorrhage,and the expression level of lncRNA MALAT1 in the poor prognosis subgroup were significantly higher than those in the good prognosis subgroup(all P<0.05).Logistic multivariate analysis results indicated Hunt-Hess gradeⅢ-Ⅳ,mFisher score 3-4,cerebral vasospasm,cerebral hemorrhage,and high level of lncRNA MALAT1 were identified as risk factors associated with poor prognosis in patients with aSAH(all P<0.05).The area under the ROC curve of serum lncRNA MALAT1 for the diagnosis of poor prognosis in aSAH patients was 0.833(95%CI:0.753-0.895),and the optimal threshold was determined to be 2.03,achieving a sensitivity of 84.91%and a specificity of 75.38%.Conclusions The expression of lncRNA MALAT1 in the serum of patients with aSAH is significantly up-regulated.This elevated expression is closely associated with the severity of the disease and patient prognosis,suggesting its potential as an auxiliary biomarker for assessing the prognosis of aSAH.
