目的 研究伴意识障碍的颞叶癫痫(TLE-IAS)患者代谢脑网络的图论属性及Hub节点的改变,探讨TLE-IAS患者特征性代谢脑网络重构的机制.方法 回顾性纳入2018-01-2025-01于兰州大学第二医院核医学科行脑 18F-FDG PET/CT检查的193例伴意识障碍颞叶癫痫患者(TLE-IAS组)及193例无中枢神经系统疾病患者(对照组).在组水平计算2组患者各脑区间的Pearson相关系数,构建TLE-IAS组及对照组的PET代谢脑网络后,采用1 000次非参数置换检验比较2组患者图论参数及Hub节点分布.结果 与对照组相比,TLE-IAS组患者大脑功能连接显著减弱;在图论参数上表现为Lp增加,Cp及平均度均显著减低(P<0.05),Eg与Eloc经FDR校正后未发现显著的组间差异点.对照组Hub节点为13个,TLE-IAS组Hub节点数量8个,与对照组相比分布出现重构,从默认模式网络向边缘系统及初级感觉皮层转移.结论 TLE-IAS患者存在广泛的脑代谢连接减弱、网络传输效率及信息处理能力受损,伴Hub节点数量减少与空间重构.

Objective To investigate the alterations in graph-theoretical properties and Hub nodes of metabolic brain networks in patients with temporal lobe epilepsy with impaired awareness seizures(TLE-IAS),and explore the mechanisms of its characteristic metabolic network reorganization.Methods A total of 193 patients(TLE-IAS group)with TLE-IAS who underwent 18F-FDG PET/CT at the Department of Nuclear Medicine,the Second Hospital of Lanzhou University between January 2018 and January 2025 were retrospectively included,along with 193 controls(control group)without central nervous system disorders.At the group level,Pearson correlation coefficients between each brain region pair were calculated for both groups to construct PET metabolic brain networks for the TLE-IAS group and the control group.Non-parametric permutation testing with 1 000 iterations was then applied to compare group differences in graph-theoretical metrics and Hub node distribution.Results Compared with the control group,globally weakened metabolic connectivity was exhibited in the TLE-IAS patient group.Graph-theoretical analysis revealed a significantly increased Lp and significantly decreased Cp and mean degree(P<0.05).However,Eg and Eloc showed no significant group differences after FDR correction.Thirteen Hub nodes were identified in the control group,compared to eight in the TLE-IAS group.Furthermore,a spatial reorganization of hubs was demonstrated in patients,characterized by a shift from the default mode network toward the limbic system and primary sensory cortices.Conclusion TLE-IAS patients exhibit widespread weakened metabolic connectivity,impaired network transmission efficiency and information processing capacity,alongside a reduced number of Hub nodes and their spatial reorganization.
