H型高血压合并AIS患者血清氨基酸类神经递质水平与认知障碍程度的相关性OA
Association between serum amino acid neurotransmitter levels and the degree of cognitive impairment in H-type hypertension patients with AIS
目的 探究H型高血压合并急性缺血性脑卒中(AIS)患者血清氨基酸类神经递质水平与认知障碍程度的相关性.方法 采用横断面研究设计,纳入2022-01-2024-01陕西省人民医院神经内一科收治的200例H型高血压合并AIS患者为研究对象.依据美国国立卫生研究院卒中量表(NIHSS)评分将纳入患者分为轻度组(≤4分,n=60)、中度组(5~20分,n=85)和重度组(>20分,n=55).采用酶联免疫吸附法测定各组患者血清谷氨酸(Glu)、天冬氨酸(Asp)、γ-氨基丁酸(GABA)、甘氨酸(Gly)水平,Spearman相关性分析各神经递质水平变化与H型高血压合并AIS患者病情的相关性.通过简易精神状态评价量表(MMSE)评估200例患者的认知功能,依据评分结果将患者分为认知障碍组(n=68)和正常组(n=132)两个亚组,建立Logistic回归模型筛选认知障碍危险因素.绘制受试者工作特征(ROC)曲线评估预测效能.结果 轻度组Glu、Asp及Gly水平显著低于中度组及重度组,GABA水平则呈反向趋势(P<0.05).Spearman相关性分析显示,Glu、Asp及Gly水平均与H型高血压合并AIS患者病情呈正相关(r=0.823、0.756、0.805),GABA 与其呈负相关(r=-0.769).Logistic 多因素回归分析显示,文化程度(OR=2.046)、卒中部位(OR=2.277)、NIHSS 评分(OR=2.248)、Glu(OR=2.974)、Asp(OR=2.210)、GABA(OR=2.418)、Gly(OR=2.237)均为H型高血压合并AIS患者发生认知功能障碍的独立影响因素(P<0.05).ROC曲线分析显示,四指标联合预测认知障碍的曲线下面积(AUC)0.920,灵敏度88.8%,特异度90.3%,显著优于单一指标.结论 血清氨基酸类神经递质水平失衡与H型高血压合并AIS患者的神经功能损伤及认知障碍进展密切相关,联合检测可为早期干预提供生物标志物支持.
Objective To investigate the association between serum amino acid neurotransmitter levels and the degree of cognitive impairment in patients with H-type hypertension complicated with acute ischemic stroke(AIS).Methods A cross-sectional study design was adopted.A total of 200 patients with H-type hypertension complicated with AIS admitted to the First Department of Neurology of Shaanxi Provincial People's Hospital from January 2022 to January 2024 were included as the research subjects.The included patients were divided into mild group(≤4 points,n=60),moderate group(5-20 points,n=85),and severe group(>20 points,n=55)based on the National Institutes of Health stroke scale(NIHSS)score.The levels of glutamate(Glu),aspartate(Asp),γ-aminobutyric acid(GABA),and glycine(Gly)in the serum of each group were determined by enzyme-linked immunosorbent assay(ELISA).Spearman correlation analysis was used to verify the correlation between the changes in neurotransmitter levels and the condition of patients with H-type hypertension complicated with AIS.The cognitive function of 200 patients was evaluated by the mini-mental state examination(MMSE),and the patients were divided into cognitive impairment group(n=68)and normal group(n=132)based on the score.Logistic regression model was established to screen the risk factors for cognitive impairment.The receiver operating characteristic(ROC)curve was drawn to evaluate the predictive efficacy.Results The levels of Glu,Asp,and Gly in the mild group were significantly lower than those in the moderate and severe groups,while the level of GABA was higher(P<0.05).Spearman correlation analysis showed that the levels of Glu,Asp,and Gly were positively correlated with the condition of patients with H-type hypertension complicated with AIS(r=0.823,0.756,0.805),while GABA was negatively correlated(r=-0.769).Multivariate Logistic regression analysis showed that educational level(OR=2.046),stroke location(OR=2.277),NIHSS score(OR=2.248),Glu(OR=2.974),Asp(OR=2.210),GABA(OR=2.418),and Gly(OR=2.237)were independent influencing factors for cognitive impairment in patients with H-type hypertension complicated with AIS(P<0.05).The ROC curve analysis showed that the area under the curve(AUC)of the combined prediction of the four indicators for cognitive impairment reached 0.920,with the sensitivity of 88.8%,the specificity of 90.3%,which was significantly better than that of a single indicator.Conclusion The imbalance of serum amino acid neurotransmitter levels is closely related to the progression of neurological function damage and cognitive impairment in patients with H-type hypertension complicated with AIS.Combined detection can provide biomarker support for early intervention.
