血运重建术治疗大面积脑梗死的研究进展OA
Research advances in revascularization surgery for the treatment of massive cerebral infarction
大面积脑梗死(MCI)作为神经外科领域的重要临床难题,特指由大脑中动脉主干或颈内动脉主干闭塞引发的广泛性脑组织缺血性坏死.目前国际学术界尚未对该病建立统一诊断标准,其发病率约占缺血性脑卒中的10%,临床特征表现为高致残性和致死性双重特性.近年来,脑血运重建技术通过改善缺血区域脑灌注、激活半暗带神经元功能,在降低TIA发生率、卒中复发风险及卒中相关病死率等方面展现出显著治疗优势.本文系统综述了近年来血运重建术治疗MCI的研究进展,重点探讨直接血运重建技术(如颞浅动脉-大脑中动脉吻合术)、间接血运重建技术(如脑-硬脑膜-颞浅动脉血管融通术)及其联合术式在MCI治疗中的临床应用进展.
Massive cerebral infarction(MCI),a critical neurosurgical challenge,refers to extensive ischemic necrosis of brain tissue caused by occlusion of the main trunk of the middle cerebral artery or internal carotid artery.The international academic community has yet to establish unified diagnostic criteria for this condition,which accounts for approximately 10%of ischemic stroke cases and is characterized by high rates of disability and mortality.In recent years,cerebral revascularization techniques have demonstrated significant therapeutic advantages in reducing transient ischemic attack(TIA)incidence,stroke recurrence risk,and stroke-related mortality through improved cerebral perfusion in ischemic regions and activation of penumbral neuronal function.This review systematically examines research advancements in revascularization strategies for MCI,with particular emphasis on the clinical applications of direct revascularization(superficial temporal artery-middle cerebral artery anastomosis,STA-MCA),indirect revascularization(encephalo-duro-arterio-synangiosis,EDAS),and combined surgical approaches.
