VMAT2抑制剂治疗亨廷顿舞蹈病的研究进展OA
Therapeutic advances of VMAT2 inhibitors in Huntington's disease
亨廷顿舞蹈病(HD)是一种以舞蹈样不自主运动、认知障碍和精神症状为主要表型的常染色体显性遗传的神经系统变性疾病,其起病隐匿、进展缓慢、预后较差、致残率较高,患者疾病负担重.目前尚缺乏成熟的疾病修饰治疗,药物干预仍以对症控制为主.囊泡单胺转运体2(VMAT2)抑制剂通过抑制突触前囊泡单胺摄取、耗竭多巴胺储备,从而减少纹状体多巴胺过度活化,是目前治疗HD患者舞蹈症的药物之一,包括丁苯那嗪、氘丁苯那嗪、缬苯那嗪.本文总结了上述3种VMAT2抑制剂在HD患者舞蹈症治疗中的药理学特点、临床证据及安全性.研究表明,3种VMAT2抑制剂均可显著改善HD患者的舞蹈症状,是目前循证证据最为充分的HD药物治疗手段之一,抑郁、自杀风险及锥体外系症状的不良反应仍需高度警惕,未来有待更多头对头试验和真实世界研究,以优化不同药物的适应人群和剂量策略,并探索其与潜在疾病修饰治疗的整合.
Huntington's disease(HD)is an autosomal dominant neurodegenerative disease,characterized clinically by choreiform involuntary movements,cognitive impairment and psychiatric symptoms,with insidious onset,slow progression,poor prognosis,high rate of disability,and a heavy disease burden.At present,no mature disease-modifying therapy is available,and pharmacological management remains largely symptomatic.Vesicular monoamine transporter 2(VMAT2)inhibitors reduce excessive dopaminergic overactivity in the striatum by inhibiting presynaptic vesicular monoamine uptake and depleting dopamine stores,and have become one of the main drug options for the treatment of chorea in patients with HD,including tetrabenazine,deutetrabenazine and valbenazine.This review summarizes the pharmacological properties,clinical evidence and safety profiles of these VMAT2 inhibitors.Current studies indicate that all three VMAT2 inhibitors can significantly improve choreic symptoms in patients with HD.However,adverse effects such as depression,increased suicide risk and extrapyramidal symptoms still warrant close attention.Future head-to-head trials and real-world studies are needed to further optimize patient selection and dosing strategies for different agents and to explore their integration with potential disease-modifying therapies.
