基于动态神经肌肉稳定技术的核心稳定性训练对脑卒中后偏瘫患者康复效果的影响OA
Effect of core stability training based on dynamic neuromuscular stabilization technique on the rehabilitation of hemiplegic patients after stroke
目的 探讨基于动态神经肌肉稳定技术(DNS)的核心稳定性训练对脑卒中后偏瘫患者的康复效果.方法 选取2023-01-2024-06承德市中心医院治疗的122例脑卒中后偏瘫患者,依据随机数表法分为常规训练组(n=61)和DNS训练组(n=61).常规训练组采用核心稳定性康复训练,DNS训练组采取基于DNS的核心稳定性训练.康复训练3个月后,比较2组患者躯体障碍改善情况[躯干功能障碍量表(TIS)]、平衡能力[Berg平衡量表(BBS)]、姿势控制能力[脑卒中患者姿势控制量表(PASS)]、肢体运动功能[Fugl-Meyer运动功能量表(FMA)]、日常生活能力[改良的Barthel指数量表(MBI)]和步行能力[10米步行测试(10MWT)、6分钟步行试验(6MWT)].结果 康复训练后,DNS训练组TIS量表中静态坐位平衡、动态坐位平衡、协调能力评分高于常规训练组(P<0.05),DNS训练组BBS评分、PASS量表中姿势维持和姿势变换评分高于常规训练组(P<0.05),DNS训练组上肢FMA、下肢FMA、MBI评分均高于常规训练组(P<0.05),10MWT短于常规训练组,6MWT距离长于常规训练组(P<0.05).结论 基于DNS的核心稳定性训练可进一步改善脑卒中后偏瘫患者的躯体障碍,提升机体的平衡能力和肢体控制能力,进而改善肢体运动功能和步行能力,提高患者日常生活能力.
Objective To investigate the rehabilitation effect of core stability training based on dynamic neuromuscular stabilization(DNS)in hemiplegia patients after stroke.Methods A total of 122 patients with hemiplegia after stroke admitted to Chengde Central Hospital from January 2023 to June 2024 were selected,and they were divided into routine training group(n=61)and DNS training group(n=61)based on the random number table method.The routine training group received the core stability rehabilitation training,and the DNS training group received the core stability training based on DNS.After 3 months of rehabilitation training,the improvement of physical disorders(trunk impairment scale,TIS),balance capacity(Berg balance scale,BBS),posture control(postural assessment scale for stroke patients,PASS),limb motor function(Fugl-Meyer assessment,FMA),the ability of daily living(modified Barthel index,MBI),walking ability(10 m walking test,10MWT;6 min walking test,6MWT)were compared between the two groups.Results After rehabilitation training,the scores of static sitting balance,dynamic sitting balance and coordination ability in TIS scale in the DNS training group were higher than those in the routine training group(P<0.05).The scores of BBS,posture maintenance and posture change in PASS scale in the DNS training group were higher than those in the routine training group(P<0.05).The upper limb FMA,lower limb FMA and MBI scores in the DNS training group were higher than those in the routine training group(P<0.05).The 10MWT in the DNS training group was shorter than that in the routine training group,and 6MWT was longer than that in the routine training group(P<0.05).Conclusion Core stability training based on DNS can further improve the physical disorders of patients with hemiplegia after stroke,improve the body's balance ability and limb control ability,and then improve the limb motor function and walking ability,and improve the patients'daily living ability.
