沿基底沟荷包缝合法包埋阑尾残端在腹腔镜阑尾切除术中的应用OA

Objective:To investigate the clinical application value of burying the appendiceal stump with a purse-string suture along the appendiceal basal groove in laparoscopic appendectomy(LA).Methods:A retrospective study was conducted on clinical data of 116 patients with acute appendicitis who underwent LA with purse-string suture along the appendiceal basal groove for stump burial from Jan.2024 to Dec.2024.During surgery,after transection of the mesoappendix and full mobilization of the appendix,the appendix was lifted and depressed to reveal a shallow groove on the cecal wall,located approximately 0.5-1.0 cm from its base;this anatomical structure was defined as the"appendiceal basal groove".Following double ligation of the appendix root,a seromuscular purse-string su-ture was performed on the cecum along the outer edge of this basal groove using 3-0 absorbable suture.The suture was sequentially placed in four stitches in a counterclockwise sequence:left anterior→ right anterior→right posterior→left posterior,to complete the burial of the stump.Results:Stump burial using the basal groove technique was successful in 113 cases,with 3 cases of incomplete burial.The average operation time was(67.4±28.9)min,the average time to postoperative flatus was(1.8±0.7)d,the average postoperative hospital stay was(4.9±1.5)d,and the average hospitalization cost was(9273.3±1593.5)CNY.Postoperative complications occurred in two patients:one case of trocar site bleeding and one case of trocar site infection.At the 1-,3-,and 6-month postoperative follow-ups,all patients recovered well without complaints of chronic abdominal pain or other significant discomforts.Conclusions:In LA,the tech-nique of burying the appendiceal stump with a purse-string suture along the appendiceal basal groove is safe,effective,and easy to per-form,warranting broader clinical application.
