Association between sleep duration trajectories and hearing loss in the China health and retirement longitudinal study
目的 基于中国健康与养老追踪调查(China Health and Retirement Longitudinal Study,CHARLS)数据,探讨社区人群睡眠时长轨迹与听力损失的关联.方法 纳入2011~2018年CHARLS四期调查数据,采用组轨迹模型(group-based trajectory model,GBTM)识别睡眠时长轨迹类型,并运用Logistic回归分析不同轨迹与听力损失的关联,调整人口学因素及代谢相关混杂因素.结果 轨迹模型拟合指标随轨迹数增加逐渐下降,但五轨迹模型平均后验概率(average posterior probability,APP)<0.7,综合信息准则变化幅度,最终确定3轨迹模型.GBTM将睡眠时长分为健康睡眠增长组(583例)、持续睡眠短睡组(1 175例)和波动睡眠不足组(1 403例),三组在人口学及代谢特征上差异显著.Logistic回归显示,调整心血管代谢相关因素后,持续短睡组听力损失风险升高(OR=1.321,P<0.05),且在≥59岁人群中更明显(OR=1.491,P=0.018),<59岁组无统计学意义.结论 持续睡眠短睡轨迹与听力损失显著相关,且在老年人群中风险更高,提示长期睡眠不足可能是老年听力损失的危险因素.
OBJECTIVE To investigate the association between sleep duration trajectories and hearing loss among community-dwelling adults using data from the China Health and Retirement Longitudinal Study(CHARLS).METHODS Data were derived from four waves(2011-2018)of the CHARLS.Group-based trajectory modeling(GBTM)was employed to identify distinct sleep duration trajectories over time.Multivariable logistic regression analyses were then performed to evaluate the associations between different sleep duration trajectory groups and hearing loss,adjusting for demographic characteristics and metabolic-related confounding factors.RESULTS Model fit indices decreased with an increasing number of trajectory groups;however,the five-group model showed an average posterior probability(APP)<0.7.Based on overall model performance,a three-trajectory model was selected.Sleep duration patterns were classified as healthy increas sleep trajectory(n=583),persistent short sleep(n=1,175),and fluctuating insufficient sleep(n=1,403).Significant differences in demographic and metabolic characteristics were observed among the three groups.After adjustment for cardiometabolic factors,persistent short sleep was significantly associated with an increased risk of hearing loss(OR=1.321,P<0.05).The association was more pronounced in participants aged≥59 years(OR=1.491,P=0.018),whereas no significant association was observed in those aged<59 years.CONCLUSION Persistent short sleep trajectory is significantly associated with hearing loss,particularly among older adults.Chronic sleep deprivation may represent an important risk factor for age-related hearing loss.
