中药烫熨联合杵针疗法对中风后偏瘫患者运动功能及日常生活能力的影响:一项随机对照研究OA
Effects of Herbal Hot Compress Combined with Chuzhen Therapy on Motor Function and Activities of Daily Living in Patients with Hemiplegia after Stroke:A Randomized Controlled Trial
目的:探讨中药烫熨联合杵针疗法对中风后遗症患者的疗效.方法:选择2024年9月～2025年8月我院针灸康复科住院治疗的中风后偏瘫患者60例,采用简单随机数字表法分为观察组和对照组各30例.对照组给予常规护理联合常规康复运动治疗,观察组在对照组治疗基础上加用中药烫熨、杵针联合情志护理治疗,治疗周期为2周.采用Fugl-Meyer量表评估运动功能,Barthel指数量表评估日常生活能力,Berg平衡量表评估平衡功能,抑郁自评量表评估心理状况.结果:治疗2周后,观察组FMA评分为72.48±10.35分,显著高于对照组的61.73±9.82分(P<0.01);观察组BI评分为71.25±15.42分,显著高于对照组的58.33±13.76分(P<0.01);观察组BBS评分为44.26±7.92分,显著高于对照组的37.42±6.75分(P<0.01);观察组SDS评分为48.52±8.74分,显著低于对照组的58.73±9.15分(P<0.01).各项指标改善幅度观察组均显著大于对照组(P<0.01).结论:中药烫熨、杵针联合情志护理能够显著改善中风后偏瘫患者的运动功能、日常生活能力、平衡功能和心理状况,为中西医结合康复模式提供了循证医学依据.
Objective:To investigate the therapeutic effect of herbal hot compress combined with Chuzhen therapy on patients with sequelae of stroke(hemiplegia after stroke).Methods:Sixty hospitalized patients with post-stroke hemiplegia who were treated in the Department of Acupuncture and Rehabilitation of our hospital from September 2024 to August 2025 were selected.Using a simple random number table,the patients were randomly divided into an observation group and a control group,with 30 patients in each group.The control group received routine nursing care combined with conventional rehabilitation exercise therapy.On the basis of the treatment for the control group,the observation group was additionally given herbal hot compress,Chuzhen therapy,and emotional care.The treatment period was 2 weeks.The Fugl-Meyer Assessment(FMA)was used to evaluate motor function,the Barthel Index(BI)to evaluate activities of daily living,the Berg Balance Scale(BBS)to evaluate balance function,and the Self-Rating Depression Scale(SDS)to evaluate psychological status.Results:After 2 weeks of treatment,the FMA score of the observation group was 72.48±10.35,which was significantly higher than that of the control group(61.73±9.82,P<0.01).The BI score of the observation group was 71.25±15.42,which was significantly higher than that of the control group(58.33±13.76,P<0.01).The BBS score of the observation group was 44.26±7.92,which was significantly higher than that of the control group(37.42±6.75,P<0.01).The SDS score of the observation group was 48.52±8.74,which was significantly lower than that of the control group(58.73±9.15,P<0.01).The improvement degree of all indicators in the observation group was significantly greater than that in the control group(P<0.01).Conclusion:Herbal hot compress combined with Chuzhen therapy and emotional care can significantly improve the motor function,activities of daily living,balance function and psychological status of patients with hemiplegia after stroke,which provides evidence-based medical support for the integrated traditional Chinese and Western medicine rehabilitation model.
