Basic Research Progress of Repetitive Transcranial Magnetic Stimulation in the Treatment of Post-stroke Cognitive Impairment
脑卒中后认知障碍(Post-stroke Cognitive Impairment,PSCI)是脑卒中后常见的主要功能障碍之一,其病理机制复杂,涉及皮层功能障碍、脑血流灌注不足、炎症反应、神经递质紊乱及"菌群-肠-脑轴"失调等多方面因素.重复经颅磁刺激(Repetitive Transcranial Magnetic Stimulation,rTMS)作为一种无痛、无创的神经调控技术,在改善PSCI方面展现出广泛的应用潜力.基础研究表明,rTMS可通过调节大脑皮层兴奋性与突触可塑性、促进脑血流和血脑屏障修复、调控神经递质及关键信号蛋白表达、调节肠道菌群等途径,多机制协同改善认知功能.然而,rTMS的具体作用机制及最优治疗参数尚未完全明确,这在一定程度上限制了其在PSCI中的临床推广.本文综述了PSCI的发病机制、rTMS的作用机制及其临床应用的基础研究进展,旨在为rTMS在PSCI治疗中的临床应用及后续研究提供参考.
Post-stroke cognitive impairment(PSCI)is one of the most common and serious functional disorders after stroke.Its pathophysiological mechanism is complex and multifactorial,mainly involving cortical dysfunction,cerebral hypoperfusion,inflammatory response,neurotransmitter imbalance,and microbiota-gut-brain axis disorder.As a non-invasive and painless neuromodulation technique,repetitive transcranial magnetic stimulation(rTMS)has shown great potential in improving cognitive function in patients with PSCI.Basic studies have revealed that rTMS may improve cognitive function by regulating cortical excitability and synaptic plasticity,promoting cerebral blood flow and blood-brain barrier repair,regulating the expression of neurotransmitters and key signaling proteins,and affecting the composition of gut microbiota.However,the unclear specific mechanism and optimal stimulation parameters of rTMS limit its further clinical application in PSCI.This article reviews the pathogenesis of PSCI,the mechanism of rTMS,and the research progress of its basic and clinical application,so as to provide a reference for the clinical application and future research of rTMS in the treatment of PSCI.
