火针围刺联合复方泽漆冲剂治疗斑块型银屑病临床观察OA
Clinical Effect of Fire Needle Surrounding Acupuncture Combined with Compound Zeqi Granule in Treatment of Plaque Psoriasis
目的 观察火针围刺联合复方泽漆冲剂治疗斑块型银屑病的临床疗效及皮肤镜下变化.方法 将66例斑块型银屑病血热证患者,通过随机数字表法分为观察组与对照组,每组33例.对照组给予安慰针联合复方泽漆冲剂口服,观察组予以火针围刺联合复方泽漆冲剂口服.连续治疗8周,比较两组治疗前及治疗2、4、6、8周后银屑病皮损面积及严重程度指数(psoriasis area and severity index,PASI)评分,血热证评分,皮肤病生活质量指数(der-matology life quality index,DLQI)评分,汉密尔顿焦虑量表(Hamilton anxiety rating scale,HAMA)评分,皮肤镜下血管球数变化情况和治疗2、4、6、8周后PASI50、PASI75、PASI90应答率,以及临床疗效和不良反应发生情况.结果 治疗4、6、8周后,两组PASI评分、血热证评分及皮肤镜下血管球数均显著下降(P＜0.05),且观察组下降幅度显著大于对照组(P＜0.05).治疗4、6周后,观察组PASI50应答率显著高于对照组(P＜0.05);治疗8周后,观察组PASI50及PASI75应答率均显著优于对照组(P＜0.05).治疗6、8周后,两组DLQI、HAMA评分均显著下降,且观察组下降更显著(P＜0.05).观察组临床疗效优于对照组(P＜0.05).治疗期间两组均未发生严重不良反应.结论 火针围刺联合复方泽漆冲剂能有效治疗斑块型银屑病,改善患者血热证候,提高生活质量,缓解焦虑情绪,减少皮肤镜下血管球数.
Objective To investigate the clinical effect of fire needle surrounding acupuncture combined with Compound Zeqi Granule in the treatment of plaque psoriasis,as well as dermoscopic changes.Methods A total of 66 patients with plaque psoriasis with blood heat syn-drome were divided into observation group and control group using a random number table,with 33 patients in each group.The patients in the control group were given placebo acupuncture combined with oral administration of Compound Zeqi Granule,and those in the observa-tion group were given fire needle surrounding acupuncture combined with oral administration of Compound Zeqi Granule.Both groups were treated for 8 consecutive weeks,and the two groups were compared in terms of Psoriasis Area and Severity Index(PASI)score,the change in the number of vascular globules under dermoscopy,blood heat syndrome score,Dermatology Life Quality Index(DLQI)score,and Hamilton Anxiety Rating Scale(HAMA)score before treatment and at weeks 2,4,6,and 8 of treatment,as well as clinical outcomes and adverse reaction.Compared PASI50/PASI75/PASI90 response rates of both groups at 2,4,6 and 8 of treatment.Results At weeks 4,6,and 8 of treatment,both groups had significant reductions in PASI score,blood heat syndrome score,and the number of vascular globules(P＜0.05),and the observation group had significantly greater reductions than the control group(P＜0.05).At weeks 4 and 6 of treatment,the observation group had a significantly higher PASI50 response rate than the control group(P＜0.05),and at week 8 of treatment,the observation group had significantly higher PASI50 and PASI75 response rates than the control group(P＜0.05).At weeks 6 and 8 of treatment,both groups had significant reductions in DLQI and HAMA scores,and the observation group had significantly greater reductions than the control group(P＜0.05).The observation group had a significantly better clinical outcome than the control group(P＜0.05).There were no serious adverse events in ei-ther group during treatment.Conclusion Fire needle surrounding acupuncture combined with Compound Zeqi Granule has a marked effect in the treatment of plaque psoriasis and can improve blood heat syndrome,enhance quality of life,alleviate anx-ious emotions,and reduce the number of vascular globules under dermoscopy.
