李氏定位旋转复位法治疗椎动脉型颈椎病伴颈椎曲度异常临床观察OA
Effect of Li's Positioning and Rotary Reduction Technique on Cervical Spondylosis of Vertebral Artery Type with Cervical Curvature Abnormality:A Study Based on the Tendon-Bone Manipulation Theory
目的 基于"筋骨并举"理论探究李氏定位旋转复位法对椎动脉型颈椎病(cervical spondylosis of vertebral artery type,CSA)伴颈椎曲度异常患者椎动脉血流动力学指标、颈椎曲度、眩晕程度的影响.方法 选取60例CSA伴颈椎曲度异常患者,采用随机数字表法分为观察组30例和对照组30例.对照组患者采用颈椎牵引联合常规推拿治疗,观察组患者在此基础上加用李氏定位旋转复位法治疗.采用经颅彩色多普勒超声诊断仪监测两组患者治疗前后两侧椎动脉收缩期峰值血流速度(Vs)、舒张期血流速度(Vd),根据颈椎侧位片测量Borden值,采用颈性眩晕症状与功能评估量表(evaluation scale for cervical vertigo,ESCV)评价眩晕程度.比较两组临床疗效.结果 治疗后两组患者双侧椎动脉Vs、Vd,Borden值,ESCV评分均显著提高(P<0.05);且观察组双侧椎动脉Vs、Vd,Borden值,ESCV评分提高幅度均显著大于对照组(P<0.05).观察组临床疗效显著优于对照组(P<0.05).结论 基于"筋骨并举"理论的李氏定位旋转复位法能有效改善CSA伴颈椎曲度异常患者的椎动脉血流动力学指标、颈椎曲度及眩晕程度.
Objective To investigate the effect of Li's positioning and rotary reduction technique on hemodynamic parameters of verte-bral artery,cervical curvature,and severity of vertigo in patients with cervical spondylosis of vertebral artery type(CSA)and cervical curvature abnormality based on the tendon-bone manipulation theory.Methods A total of 60 patients with CSA and cervical curvature abnormality were enrolled and divided into observation group with 30 patients and control group with 30 patients using a random num-ber table.The patients in the control group received cervical traction combined with routine massage,while those in the observation group received Li's positioning and rotary reduction technique in addition to the treatment in the control group.A transcranial Doppler blood flow analyzer was used to monitor peak systolic blood flow velocity(Vs)and diastolic blood flow velocity(Vd)of bilateral verte-bral arteries before and after treatment;the Borden value was measured based on the lateral film of the cervical vertebra;Evaluation Scale for Cervical Vertigo(ESCV)was used to assess the severity of vertigo.Clinical outcome was compared between the two groups.Results After treatment,both groups had significant increases in Vs and Vd of bilateral vertebral arteries,Borden value,and ESCV score(P<0.05),and the observation group had significantly greater increases in these indicators than the control group(P<0.05).The observation group had a better clinical outcome than the control group(P<0.05).Conclusion Li's positioning and rotary reduc-tion technique based on the tendon-bone manipulation theory can effectively improve the hemodynamic parameters of vertebral artery,cervical curvature,and the severity of vertigo in patients with CSA and cervical curvature abnormality.
