基于方证对应探讨凉血消风散干预特应性皮炎风湿热蕴证的机制OA
Exploration on Mechanism of Liangxue Xiaofeng Powder in Intervening Atopic Dermatitis with Wind-Damp-Heat Accumulation Pattern Based on Formula-Pattern Correspondence
目的 探讨凉血消风散治疗特应性皮炎风湿热蕴证的作用机制,从方证对应角度重点分析其剂量依赖性调控Th1/Th2免疫平衡及血清炎性因子的效应.方法 30只SD雌性大鼠依据随机数字表随机分为空白组(Control组)、特应性皮炎证型组(AD-JB组)、凉血消风散低剂量组(LXXFS-L组)、凉血消风散中剂量组(LXXFS-M组)和凉血消风散高剂量组(LXXFS-H组).除Control组外,其余各组均采用7%二硝基氯苯(DNCB)丙酮溶液诱导特应性皮炎模型,同时给予风湿热刺激,共14 d.造模第15天,LXXFS-L、M、H组分别给予凉血消风散1.4、2.8、5.6 g/mL灌胃,Control组和AD-JB组灌胃等量生理盐水,连续灌胃14 d.干预结束后,检测各组大鼠血清中细胞因子IL-4、IL-6及免疫球蛋白IgE水平,同时取大鼠背部病变皮肤制作病理切片,进行HE染色和免疫荧光检测.结果 ①剂量依赖性抑制炎性因子(ELISA):LXXFS-H组能显著降低血清IL-4、IL-6、IgE水平(P<0.01);②剂量依赖性恢复免疫平衡(免疫荧光):AD-JB组中INF-γ/IL-4的表达比值较低,LXXFS-H组INF-γ/IL-4的表达比值较其显著升高;③疗效梯度:低剂量组<中剂量组<高剂量组.结论 凉血消风散通过剂量依赖性调控Th1/Th2免疫失衡及血清炎性因子释放,实现对AD风湿热蕴证的阶梯式治疗效应,为中医方证对应理论指导下的个体化用药提供了实验依据.
Objective To explore the mechanism of action of Liangxue Xiaofeng Powder in treating atopic dermatitis(AD)with wind-damp-heat accumulation pattern,with a focus on analyzing its dose-dependent effect on regulating Th1/Th2 immune balance and serum inflammatory factors from the perspective of formula-pattern correspondence.Methods A total of 30 SD rats were divided into control group,atopic dermatitis with wind-damp-heat accumulation pattern group(AD-JB group),low-dose Liangxue Xiaofeng Powder group(LXXFS-L group),medium-dose Liangxue Xiaofeng Powder group(LXXFS-M group)and high-dose Liangxue Xiaofeng Powder group(LXXFS-H group).Excluding the control group,all other groups were administered with a 7%dinitrochlorobenzene(DNCB)acetone solution to induce the atopic dermatitis model,and received wind-damp-heat stimulation simultaneously for 14 days.On the 15th day of modeling,LXXFS-L,M,and H groups were administered Liangxue Xiaofeng Powder at doses of 1.4,2.8,and 5.6 g/mL by gavage respectively,while control group and AD-JB group were given an equal volume of normal saline by gavage respectively.The gavage lasted for 14 consecutive days.After the intervention,serum levels of cytokines IL-4,IL-6,and immunoglobulin IgE of rats in each group were detected;The lesional skin on the back of rats was collected and processed into pathological sections for HE staining and immunofluorescence detection.Results ①Dose-dependent inhibition of inflammatory factors(detected by ELISA)was observed:serum levels of cytokines IL-4,IL-6,and immunoglobulin IgE reduced significantly in the LXXFS-H group(P<0.01).②There was a dose-dependent restoration of immune balance(detected by immunofluorescence):the expression ratio of INF-γ/IL-4 was relatively low in the AD-JB group,while the ratio of INF-γ/IL-4 in the LXXFS-H group was significantly higher than that in the AD-JB group.③The therapeutic effect showed a gradient of LXXFS-L group<LXXFS-M group<LXXFS-H group.Conclusion Liangxue Xiaofeng Powder exerts a stepped therapeutic effect on AD with wind-damp-heat accumulation pattern,by dose-dependently regulating Th1/Th2 immune imbalance and the release of serum inflammatory factors.This provides experimental evidence for the TCM theory of"formula-pattern correspondence"in guiding individualized medication.
