Study on Mechanism of Action of Zelan Mixture in Regulating Synovial Inflammatory Response and Coagulation Function after Knee Joint Injury in Knee Osteoarthritis Model

目的 基于炎症反应及凝血功能探讨泽兰合剂干预膝骨关节炎(KOA)模型大鼠在应激反应中的分子机制.方法 55只SD大鼠随机分为对照组、模型组和泽兰合剂低、中、高剂量组;除对照组外,其余大鼠采用前交叉韧带离断法建立KOA动物模型.造模成功后对照组及模型组予以等量生理盐水灌胃,泽兰合剂低、中、高剂量组给予泽兰合剂1、2、4 mL灌胃,2次/d,共干预7 d.灌胃干预后第1、3、7天收集各组SD大鼠的腹主动脉血、膝关节滑膜组织进行相关检测.采用HE染色观察SD大鼠膝关节滑膜组织情况;滑膜组织形态学评分评估滑膜组织形态学改变;RT-qPCR检测各组SD大鼠膝关节滑膜组织中炎症相关基因IL-6、TNF-α、NF-κB p65的相对表达量;Western blot检测各组SD大鼠膝关节滑膜组织中炎症相关蛋白IL-6、TNF-α、NF-κB p65的相对表达量;ELISA检测各组SD大鼠血清中IL-6、TNF-α、ICAM、VCAM、TXA2等因子的含量.结果 ①滑膜组织形态学评分:与模型组相比,泽兰合剂高剂量组滑膜组织形态学评分显著降低(P<0.05);②RT-qPCR:与模型组相比,泽兰合剂中、高剂量组滑膜组织中IL-6、TNF-α、NF-κB p65 mRNA相对表达量均显著降低(P<0.01);③Western blot:与模型组相比,泽兰合剂中、高剂量组滑膜组织中IL-6、TNF-α、NF-κB p65蛋白相对表达量均显著降低(P<0.05);④ELISA:与模型组相比,泽兰合剂低、中、高剂量组血清中术后第7天IL-6、TNF-α、ICAM、VCAM、TXA2含量显著降低(P<0.05).结论 泽兰合剂可降低KOA模型大鼠的炎症指标并改善高凝状态,调控滑膜组织中炎症相关基因、蛋白的表达及血清中炎症因子、凝血因子的水平,改善KOA模型大鼠的应激反应,其作用机制可能与降低炎症反应、改善高凝状态有关.

Objective To explore the molecular mechanism of Zelan Mixture in intervening in the stress response in knee osteoarthritis(KOA)model rats from the perspectives of inflammatory response and coagulation function.Methods Fifty-five SD rats were randomly divided into the control group,the model group,and the low-,medium-,and high-dose Zelan Mixture groups.In all groups except for the control group,KOA animal model was built by anterior cruciate ligament transection method.After successful modeling,the control group and the model group were given equivalent volume of normal saline by gavage,while the low-,medium-,and high-dose Zelan Mixture groups were given 1 mL,2 mL,and 4 mL Zelan Mixture by gavage,respectively.The administration was performed twice a day for 7 days.The abdominal aortic blood and knee synovium of SD rats in each group were collected on the first,third and seventh day after gavage intervention for relevant tests.HE staining was used to observe the condition of synovial tissue in SD rats;Synovial tissue morphology score were used to evaluate the morphological change of synovial tissue;RT-qPCR was used to detect the relative expression of inflammatory-related genes IL-6,TNF-α,and NF-κB p65 in synovial tissue of SD rats in each group;Western blot was used to detect the relative expression of inflammatory-related proteins IL-6,TNF-α,and NF-κB p65 in synovial tissue of SD rats in each group;ELISA was used to detect the content of IL-6,TNF-α,ICAM,VCAM,and TXA2 in the serum of SD rats in each group.Results ①Synovial tissue morphology score:Compared with the model group,the synovial tissue morphology score of the high-dose Zelan Mixture group was significantly lower(P<0.05);②RT-qPCR:Compared with the model group,the relative expression levels of IL-6,TNF-α,and NF-κB p65 mRNA in synovial tissue of the medium-and high-dose Zelan Mixture groups were significantly lower(P<0.01);③Western blot:Compared with the model group,the relative expression levels of IL-6,TNF-α,and NF-κB p65 protein in synovial tissue of the medium-and high-dose Zelan Mixture groups were significantly lower(P<0.05);④ELISA:Compared with the model group,the serum contents of IL-6,TNF-α,ICAM,VCAM,and TXA2 in the low-,medium-,and high-dose Zelan Mixture groups were significantly lower at seventh day after surgery(P<0.05).Conclusion Zelan Mixture can alleviate the inflammatory indicators and improve the hypercoagulable state in KOA model rats,regulate the related expression of inflammatory-related genes and proteins in synovial tissue,and regulate the levels of inflammatory factors and coagulation factors in serum,thereby ameliorating the stress response in KOA model rats.The mechanism of action may be related to the reduction of inflammatory response and improvement of hypercoagulable state.
