基于数据挖掘和网络针灸学探讨针灸治疗糖尿病前期核心腧穴的作用机制OA
Exploring Mechanism of Core Acupoints in Acupuncture and Moxibustion Treatment for Prediabetes Based on Data Mining and Network Acu-Informatics
目的:通过数据挖掘和网络针灸学的方法,总结分析针灸治疗糖尿病前期的选穴规律,并探讨其核心腧穴组合的作用机制.方法:以中国知网、万方数据库、维普数据库、中国生物医学文献数据库、PubMed、Embase、Cochrane Library及 Web of Science中收录的相关研究文献为数据来源,对针灸处方中的腧穴进行使用频数、频率统计、关联分析与核心腧穴组合分析等,然后对核心腧穴组合进行作用靶点挖掘、"核心腧穴-共有靶点"网络构建、蛋白质相互作用(PPI)网络分析、KEGG和GO富集分析以探究针灸治疗糖尿病前期的作用机制.结果:共纳入针灸处方100条,核心腧穴组合为三阴交、足三里、脾俞、丰隆和胃脘下俞,挖掘到其潜在治疗靶点96个,与糖尿病前期共有靶点50个,经PPI网络分析提取得到核心靶点,包括白细胞介素6(IL-6)、信号转导与转录激活分子3(STAT3)及白细胞介素1 β(IL-1β)等;经KEGG和GO富集分析得到核心腧穴组合干预糖尿病前期的信号通路主要为脂肪细胞因子、非酒精性脂肪肝、血脂与动脉粥样硬化、酒精性肝病及胰岛素抵抗等信号通路,参与碳水化合物代谢过程的调节、对胰岛素的反应、对肽激素的反应、对氧化水平的反应、小分子代谢过程的调控及细胞对脂质的反应等生物过程.结论:针灸治疗糖尿病前期的核心腧穴组合为三阴交、足三里、脾俞、丰隆和胃脘下俞,其通过多靶点、多通路协同发挥治疗糖尿病前期的作用.另外,对于肥胖、脂肪肝及血脂异常等代谢综合征表现的综合干预可能存在一定优势.
Objective:To summarize and analyze the acupoint selection patterns of acupuncture and moxibustion for prediabetes using data mining and network acu-informatics,and to explore the action mechanisms of core acupoint combinations.Methods:Relevant studies indexed in CNKI,Wanfang,VIP,CBM,PubMed,Embase,Cochrane Library,and Web of Science were used as data sources.The acupoints involved in acupuncture and moxibustion prescriptions were analyzed in terms of usage frequency,relative frequency statistics,association analysis,and core acupoint combination extraction.Mechanistic exploration was performed based on the identified core acupoint combinations,including target prediction,construction of a'core acupoint-shared targets'network,protein-protein interaction(PPI)network analysis,and KEGG and GO enrichment analyses,to investigate the potential mechanisms of acupuncture and moxibustion in treating prediabetes.Results:A total of 100 acupuncture prescriptions were included.The core acupoint combination consisted of Sanyinjiao(SP6),Zusanli(ST36),Pishu(BL20),Fenglong(ST40),and Weiwanxiashu(EX-B3).A total of 96 potential therapeutic targets were identified,of which 50 overlapped with targets associated with prediabetes.PPI network analysis revealed key targets,including interleukin-6(IL-6),signal transducer and activator of transcription 3(STAT3),and interleukin-1β(IL-1β).KEGG and GO enrichment analyses indicated that the core acupoint combination modulates signaling pathways related to adipocytokines,non-alcoholic fatty liver disease,lipid metabolism and atherosclerosis,alcoholic liver disease,and insulin resistance.It is involved in regulating biological processes such as carbohydrate metabolism,insulin response,peptide hormone response,oxygen level response,small-molecule metabolic processes,and cellular response to lipids.Conclusion:The core acupoint combination of acupuncture and moxibustion treatment for prediabetes includes SP6,ST36,BL20,ST40,and EX-B3.This combination exerts therapeutic effects on prediabetes through multi-target and multi-pathway synergistic mechanisms.Additionally,it may offer advantages in the comprehensive management of metabolic syndrome manifestations such as obesity,fatty liver,and dyslipidemia.
