独活寄生汤配合颈椎康复训练治疗风寒湿痹、肝肾亏虚型神经根型颈椎病的临床观察OA
Clinical Observation on Duhuo Jisheng Decoction Combined with Cervical Rehabilitation Training in the Treatment of Radiculopathy Cervical Spondylosis with Wind-Cold-Dampness Arthralgia and Liver-Kidney Deficiency Pattern
目的 观察独活寄生汤配合颈椎康复训练治疗风寒湿痹、肝肾亏虚型神经根型颈椎病的治疗效果.方法 选择符合纳入标准的风寒湿痹、肝肾亏虚型神经根型颈椎病患者,依据随机数字表随机分为治疗组和对照组,每组各52例.对照组给予颈椎康复训练治疗,观察组给予独活寄生汤配合颈椎康复训练治疗,两组均治疗1个月.比较两组患者治疗前后中医证候积分、疼痛视觉模拟(VAS)评分、颈椎功能障碍指数(NDI)、田中靖久症状体征评分表(TTKS)评分和血清炎症因子TNF-α、IL-1β、IL-6含量的变化情况,并比较临床疗效.结果 与治疗前比较,两组治疗后中医证候积分和NDI、TTKS、VAS评分均明显降低(P<0.01),血清炎症因子TNF-α、IL-1β和IL-6含量均显著下降(P<0.01);与对照组治疗后比较,治疗组中医证候积分和NDI、TTKS、VAS评分均明显降低(P<0.01),血清炎症因子TNF-α、IL-1β和IL-6含量均显著下降(P<0.01).对照组总有效率75%(39/52),明显低于对照组总有效率92.3%(48/52),两组比较差异有统计学意义(P<0.01).结论 独活寄生汤配合颈椎康复训练治疗神经根型颈椎病,可有效减轻患者疼痛,降低炎症因子水平,提高颈部功能和活动度.
Objective To observe the therapeutic effect of Duhuo Jisheng Decoction combined with cervical rehabilitation training in the treatment of radiculopathy cervical spondylosis with wind-cold-dampness arthralgia and liver-kidney deficiency pattern.Methods Patients of radiculopathy cervical spondylosis with wind-cold-dampness arthralgia and liver-kidney deficiency pattern who met the inclusion criteria were selected and randomly divided into the treatment group and the control group according to the random number table,with 52 cases in each group.The control group was treated with cervical rehabilitation training alone,while the observation group was treated with Duhuo Jisheng Decoction combined with cervical rehabilitation training.Both groups received treatment for 1 month.Before and after treatment,the changes in traditional Chinese medicine(TCM)syndrome scores,Visual Analogue Scale(VAS)scores for pain,Neck Disability Index(NDI),Yasuhisa Tanaka's Scoring System for Symptoms and Signs(TTKS)scores,and serum levels of inflammatory factors(TNF-α,IL-1β,IL-6)contents were compared between the two groups.Additionally,the clinical efficacy of the two groups was compared.Results Compared with the pre-treatment,both groups showed significant decreases in TCM syndrome scores,NDI scores,TTKS scores,VAS scores (all P＜0.01),and serum levels of TNF-α,IL-1β,and IL-6 contents(all P＜0.01)after treatment.Compared with the control group after treatment,the treatment group had significantly lower TCM syndrome scores,NDI scores,TTKS scores,VAS scores(all P＜0.01),and serum levels of TNF-α,IL-1β,and IL-6 contents(all P＜0.01).The total effective rate was 75％(39/52)in the control group,which was significantly lower than the rate of 92.3％ (48/52)in the treatment group,with a statistically significant difference between the two groups(P＜0.01).Conclusion Duhuo Jisheng Decoction combined with rehabilitation training in the treatment of radiculopathy cervical spondylosis can effectively relieve patients' pain,reduce the levels of inflammatory factors,and improve cervical function and mobility.
