孤独症谱系障碍(ASD)是一种发病机制复杂的神经发育障碍性疾病,其发病与肠道菌群紊乱及中医"脾"功能密切相关."脾为之卫"理论指出脾对人体防御意义重大,涉及运化、卫固、协调气血等功能.临床研究表明,ASD患儿普遍存在脾胃功能失调与肠道菌群紊乱,二者通过微生物-肠-脑轴交互影响,涉及神经、代谢、免疫等途径影响ASD发病.本文通过整合中医理论与现代微生物学研究,提出"脾失之卫"是ASD的核心病机之一,从脾论治可通过调节肠道菌群、改善肠屏障功能、抑制炎症反应等多途径发挥治疗作用,以"健脾固卫"为原则的中西医结合干预策略,具有多靶点、整体调节的优势.

Autism spectrum disorder(ASD)is a neurodevelopmental disorder with a complex pathogenesis,closely associated with gut microbiota dysbiosis and the function of the"spleen"in traditional Chinese medicine(TCM).The theory of"spleen acts as the defender"emphasizes the critical role of the spleen in human defense mechanisms,involving functions such as transportation and transformation,consolidation of defensive qi,and coordination of qi and blood.Clinical studies have shown that children with ASD commonly exhibit spleen-stomach dysfunction and gut microbiota imbalances.These two factors interact through the"microbiota-gut-brain axis",influencing the pathogenesis of ASD via neural,metabolic,and immune pathways.By integrating TCM theory with modern microbiological research,it is proposed that"spleen acts as the defender"is one of the core pathological mechanisms of ASD.Treatment focusing on the spleen can exert therapeutic effects through multiple pathways,including regulating gut microbiota regulation,intestinal barrier function improvement,and inflammatory responses inhibition.Based on the principle of"fortifying the spleen and consolidating defensive qi",an integrated Chinese-Western medicine intervention strategy offers the advantages of multi-target and holistic regulation.
