认知储备与帕金森病脑电活动的相关性研究OA

Associations between cognitive reserve and cerebral electrical activity in Parkinson's disease

目的 探讨帕金森病(Parkinson's disease,PD)患者认知储备与脑电活动之间的关系.方法 选取2024年6月至2025年6月安徽医科大学第三附属医院就诊的31例PD患者纳入PD组,同时选取人口学资料相匹配的健康体检者28名纳入对照组,观察两组受试者的认知储备、神经心理学及脑电各频段不同脑区域的功率谱密度和相对功率.结果 PD组患者的简易精神状态检查量表(mini-mental state examination,MMSE)评分、认知储备指数(cognitive reserve index,CRI)问卷评分、数字广度测验-顺背评分、额叶功能评定量表评分、数字符号替代测试评分均显著低于对照组,汉密尔顿焦虑量表评分、汉密尔顿抑郁量表评分均显著高于对照组(P<0.05).PD组患者的MMSE评分与CRI总分、教育年限、休闲时间均呈正相关(P<0.05).PD组患者δ波、θ波的前部区域、中央区域及后部区域功率谱密度均高于对照组(P<0.05);PD组患者的CRI总分与前部区域的δ波、θ波均呈负相关(P<0.05),与前部区域、中央区域的high-β波均呈正相关(P<0.05).结论 较高的受教育水平与充足的休闲活动可通过增强认知储备延缓PD患者的认知功能衰退;额叶区与中央-颞叶区的脑电节律改变可能与认知储备相关.

Objective To explore the associations between cognitive reserve and cerebral electrical activity in patients with Parkinson's disease(PD).Methods Thirty-one PD patients who visited the Third Affiliated Hospital of Anhui Medical University from June 2024 to June 2025 were selected and included in PD group.Meanwhile,28 healthy individuals who underwent physical examinations with matched demographic data were selected and included in control group.The cognitive reserve,neuropsychology,and the power spectral density and relative power of different brain regions in various frequency bands of electroencephalogram were assessed and compared between two groups.Results The scores of mini-mental state examination(MMSE),cognitive reserve index(CRI)questionnaire,digital span test-forward,frontal assessment battery and digt symbol substitution test in PD group were significantly lower than those in control group,while the scores of Hamilton anxiety scale and Hamilton depression scale were significantly higher than those in control group(P<0.05).The MMSE score of patients in PD group was positively correlated with the total score of CRI,years of education and leisure time(P<0.05).The power spectral densities of δ waves and θ waves in the anterior,central and posterior regions of patients in PD group were all higher than those in control group(P<0.05).The total score of CRI of patients in PD group was negatively correlated with δ wave and θ wave in the anterior region(P<0.05),and positively correlated with high-β wave in both the anterior and central regions(P<0.05).Conclusion Higher educational attainment and sufficient leisure activities can delay the cognitive decline of PD patients by enhancing cognitive reserves.The changes in electroencephalogram rhythms in the frontal and the central-temporal regions may be related to cognitive reserve.
