目的:探究钠-葡萄糖耦联转运体2(SGLT2)抑制剂对合并糖尿病急性缺血性脑卒中(AIS)患者近期神经功能的影响.方法:对2022年1月至2024年11月急性卒中质量评估与管理在线数据库(来源于CASE-Ⅱ研究,NCT04487340)登记的合并糖尿病AIS患者进行回顾性分析.根据是否使用SGLT2抑制剂将患者分为SGLT2抑制剂组和对照组.采用改良Rankin量表(mRS)评分评估患者出院时神经功能,并采用1:2倾向评分匹配(PSM)和二元logistic回归分析SGLT2抑制剂对患者出院时神经功能的影响.同时,按既往糖尿病和入院后新诊断糖尿病对患者进行分层,分析SGLT2抑制剂长期用药和短期用药对患者出院神经功能结局的影响.结果:共纳入 9220 例患者,患者的年龄为(67±11)岁,其中 3631 例(39.38%)为女性,1915例(20.77%)患者使用SGLT2抑制剂.PSM后,未使用SGLT2抑制剂3683例,使用SGLT2抑制剂1912例.PSM前,与对照组比较,SGLT2抑制剂组出院mRS评分0~2分的比例更高(分别为69.19%和79.84%,P<0.01).校正混杂因素后,使用SGLT2抑制剂与出院mRS评分 0~2分的比例(校正OR=1.24,95%CI:1.08~1.42)和mRS评分0~3分的比例(校正OR=1.62,95%CI:1.32~1.99)升高均显著相关(均P<0.01).PSM后,使用SGLT2抑制剂与患者出院时神经功能的关系与PSM前一致.分层分析结果显示,无论在既往糖尿病患者还是新诊断糖尿病患者中,使用SGLT2抑制剂与患者出院时mRS评分0~2分的比例和mRS评分0~3分的比例升高均显著相关(均P<0.05).结论:在合并糖尿病AIS患者中,使用SGLT2抑制剂与更好的出院时神经功能显著相关,提示SGLT2抑制剂可能有助于此类患者的短期神经功能恢复.

Objective:To investigate the association between the use of sodium-glucose linked transporter 2(SGLT2)inhibitors and functional outcomes at discharge in diabetic patients with acute ischemic stroke(AIS).Methods:A retrospective cohort study was conducted using data from the Acute Stroke Patients for Stroke Management Quality Evaluation online database(CASE-Ⅱ,NCT04487340)for patients admitted between January 2022 and November 2024.Diabetic patients with AIS were categorized as SGLT2 inhibitor users or non-users(control group).Patients'functional outcomes were assessed using the modified Rankin Scale(mRS)score.Propensity score matching(PSM)was performed in a 1∶2 ratio.Multivariate logistic regression models were used to assess the association between SGLT2 inhibitor use and outcomes.Stratified analyses were performed based on pre-existing diabetes versus newly diagnosed diabetes during the hospitalization and the impact of long-term and short-term use of SGLT2 inhibitors on patients'functional outcomes at discharge was analyzed.Results:The study included 9220 patients(mean age 67±11 years;39.38%female),of whom 1915(20.77%)were SGLT2 inhibitor users.There were 1912 cases in the user group and 3683 cases in the non-user group after PSM.Before PSM,the proportion of patients with an mRS score 0-2 at discharge was significantly higher in the SGLT2 inhibitor group than that in the control group(79.84%vs.69.19%,P<0.01).After multivariable adjustment,SGLT2 inhibitor use remained significantly associated with higher proportion of mRS score 0-2(adjusted OR=1.24,95%CI:1.08-1.42,P<0.01)and mRS score 0-3(adjusted OR=1.62,95%CI:1.32-1.99,P<0.01)at discharge.Results were consistent after PSM.In stratified analyses,SGLT2 inhibitor use was associated with improved functional outcomes in both patients with pre-existing diabetes and those with newly diagnosed diabetes.Conclusion:Among diabetic patients with AIS,SGLT2 inhibitor use is independently associated with better functional outcomes at discharge,showing a potential benefit for short-term neurological recovery.
