固元通脉汤对肾虚血瘀型阳痿患者及舌苔菌群微生态的影响OA
Effects of Guyuan Tongmai decoction on erectile dysfunction of kidney deficiency and blood stasis pattern and the microecology of tongue coating microbiota
目的 初步探讨固元通脉汤对肾虚血瘀型阳痿患者的疗效及对舌苔菌群微生态的影响.方法 选取贵州中医药大学第一附属医院2022年9月至2024年9月收治的20例无肾虚血瘀型阳痿患者为对照组(仅提供健康状态下的舌苔菌群);选取同期20例诊断为肾虚血瘀型阳痿患者为病例组,并予固元通脉汤治疗.采集对照组及病例组治疗前后舌苔菌群标本,运用16S核糖体核糖核酸基因测序获得各组舌苔微生物学信息.运用彩色多普勒超声采集病例组治疗前后的阴茎血流动力学数据(收缩期峰值流速、舒张末期流速、阻力指数).比较病例组治疗前后的国际勃起功能指数-5评分、中医证候积分、阴茎血流动力学指标及各组舌苔菌群微生物差异情况.结果 与治疗前比较,治疗后国际勃起功能指数-5评分显著改善(P<0.01),中医证候积分明显降低(P<0.01),收缩期峰值流速、阻力指数均升高(P<0.01),舒张末期流速降低(P<0.05).在属水平上,对照组和病例组治疗前后样本菌属相对丰度前20的物种差异无统计学意义(P>0.05);热图结果显示治疗后物种丰度较治疗前升高.线性判别分析效应量结果显示,在属水平上,病例组治疗前丰度较高的菌属为韦荣球菌属,对照组丰度较高的菌属为弯曲杆菌属,病例组治疗后丰度较高的菌属为酒球菌属.结论 固元通脉汤可有效改善肾虚血瘀型阳痿患者的国际勃起功能指数-5评分、中医证候积分及阴茎血流动力学指标;固元通脉汤可有效改善排名前20的舌苔菌群相对丰度.
Objective Preliminary study on the efficacy of Guyuan Tongmai decoction in patients with erectile dysfunction of kidney deficiency and blood stasis pattern and its impact on tongue coating microflora ecology.Methods Twenty healthy males admitted to the First Affiliated Hospital of Guizhou University of Traditional Chinese Medicine from September 2022 to September 2024 were selected as control group(only providing tongue coating microbiota in a healthy state);Concurrently,20 patients with erectile dysfunction of the kidney deficiency and blood stasis pattern were selected as patient group and treated with Guyuan Tongmai decoction.Tongue coating microbiota samples were collected from both groups.Microbial information from these samples was obtained using 16S ribosomal RNA gene sequencing(16S rRNA sequencing).Penile hemodynamic parameters,including peak systolic velocity(PSV),end-diastolic velocity(EDV),and resistance index(RI)were measured and recorded via color doppler ultrasound before and after treatment.Observations were made on the following parameters before and after treatment:The international index of erectile function-5(IIEF-5)score,traditional Chinese medicine syndrome score(TCMS),penile hemodynamic indicators,and the differences in tongue coating microbiota composition across the groups.Results Compared with pretreatment,post-treatment results demonstrated:IIEF-5 scores showed significant improvement(P<0.01);TCMS were significantly reduced(P<0.01);Penile hemodynamic parameters exhibited the following changes:PSV and RI significantly increased(P<0.01);EDV significantly decreased(P<0.05).Genus-level analysis showed no significant differences(P>0.05)in the top 20 genera by relative abundance among control group,pre-and post-treatment of patient group.Heatmap visualization,post-treatment samples exhibited significantly increased microbial abundance compared to pre-treatment state.Significantly enriched genera of linear discriminant analysis effect size results showed that control group:Campylobacter;Pre-treatment of patient group:Veillonella;Post-treatment of patient group:Oenococcus.Conclusion Guyuan Tongmai decoction significantly improves the IIEF-5 scores,TCMS,and penile hemodynamic parameters in patients with erectile dysfunction of the kidney deficiency and blood stasis pattern.Guyuan Tongmai decoction significantly modulates the relative abundance of the top 20 most abundant bacterial genera in tongue coating microbiota.
