流式细胞仪检测外周血IL-8、IL-17水平对三阴性乳腺癌术前淋巴结转移的预测价值OA
Predictive Value of Peripheral Blood IL-8 and IL-17 Levels Detected by Flow Cytometry for Preoperative Lymph Node Metastasis of Triple-Negative Breast Cancer
目的 探讨外周血白细胞介素-8(IL-8)、白细胞介素-17(IL-17)水平对三阴性乳腺癌(TNBC)患者术前淋巴结转移的预测价值.方法 选取2023 年9 月至 2025 年 1 月本院乳腺外科收治的 110 例TNBC患者为研究对象,根据术后病理结果分为淋巴结转移组(42 例)和无淋巴结转移组(68 例).采用流式细胞仪检测两组患者术前外周血IL-8、IL-17 水平,收集临床病理资料并进行单因素、多因素Logistic回归分析,绘制受试者工作特性(ROC)曲线评估IL-8、IL-17 单独及联合检测的预测效能.结果 淋巴结转移组的外周血IL-8、IL-17 水平显著高于无淋巴结转移组(P<0.001).单因素分析显示,肿瘤直径、组织学分级、癌胚抗原(CEA)、糖类抗原 15-3(CA15-3)与TNBC术前淋巴结转移相关(P<0.05).多因素Logistic回归分析表明,外周血IL-8、IL-17 水平是TNBC术前淋巴结转移的独立危险因素(P<0.001).ROC曲线显示,IL-8、IL-17 单独检测的AUC分别为0.713、0.744,联合检测的AUC为 0.857,联合预测效能更高.结论 外周血IL-8、IL-17 水平升高与TNBC术前淋巴结转移密切相关,二者联合检测可提高对淋巴结转移的预测准确性,为临床制定手术方案及评估预后提供依据.
Objective To explore the predictive value of peripheral blood interleukin-8(IL-8)and interleukin-17(IL-17)levels for preoperative lymph node metastasis in patients with triple-negative breast cancer(TNBC).Methods 110 TNBC patients admitted to the breast surgery department of our hospital from September 2023 to January 2025 were selected as study subjects.They were divided into the lymph node metastasis group(42 cases)and the non-lymph node metastasis group(68 cases)based on postoperative pathological results.Preoperative peripheral blood IL-8 and IL-17 levels were detected using flow cytometry in both groups.Clinical pathological data were collected and subjected to univariate and multivariate Logistic regression analysis.Receiver operating characteristic(ROC)curves were plotted to evaluate the predictive performance of IL-8,IL-17,and their combined detection.Results The levels of IL-8 and IL-17 in peripheral blood of the lymph node metastasis group were significantly higher than those of the non-lymph node metastasis group(P<0.001).Univariate analysis showed that tumor diameter,histological grade,carcinoembryonic antigen(CEA),and carbohydrate antigen 15-3(CA15-3)were associated with preoperative lymph node metastasis in TNBC(P<0.05).Multivariate Logistic regression analysis indicated that the levels of IL-8 and IL-17 in peripheral blood were independent risk factors for preoperative lymph node metastasis of TNBC(P<0.001).ROC curve analysis revealed that the area under the curve(AUC)for individual detection of IL-8 and IL-17 was 0.713 and 0.744,respectively,while the AUC for combined detection was 0.857;the combined predictive efficacy was higher.Conclusions Elevated levels of IL-8 and IL-17 in peripheral blood are closely associated with preoperative lymph node metastasis of TNBC.Combined detection of these two markers can improve the accuracy of predicting lymph node metastasis,providing a basis for clinical surgical planning and prognosis evaluation.
