目的 分析神经肌电图在2型糖尿病(T2DM)患者糖尿病周围神经病变(DPN)早期诊断及病情进展评估中的应用价值.方法 选取 2024年1 月至2025年 6月本院收治的168例T2DM患者,根据是否合并DPN分为观察组(98例,合并DPN)和对照组(70 例,未合并DPN),另将同期于本院行体检的 84 例健康体检者纳入健康组,均接受神经肌电图检查.测量并比较三组的正中神经、尺神经、腓总神经的运动神经传导速度(MNCV)、感觉神经传导速度(SNCV);绘制受试者工作特征(ROC)曲线,分析神经肌电图相关参数诊断T2DM患者DPN的价值;另根据DPN患者有无症状分为有症状组(共 56 例)及无症状组(共42例),检测并对比两组的神经肌电图相关参数.结果 与对照组、健康组相比,观察组各神经的MNCV与SNCV均更低(P<0.05).ROC曲线显示:正中神经、尺神经、腓总神经的MNCV与SNCV联合诊断T2DM患者DPN的曲线下面积高于各项指标单独诊断(P<0.05).与无症状组相比,有症状组各神经的MNCV与SNCV均更低(P<0.05).结论 神经肌电图能有效诊断T2DM患者的DPN,还可对其病情进展进行良好评估,有一定的应用价值.

Objective To analyze the application value of electromyography in the early diagnosis and progression assessment of diabetic peripheral neuropathy(DPN)in patients with type 2 diabetes mellitus(T2DM).Methods A total of 168 T2DM patients admitted to our hospital from January 2024 to June 2025 were selected and divided into the observation group(98 cases,with DPN)and the control group(70 cases,without DPN)based on the presence of DPN.Additionally,84 healthy individuals undergoing physical examinations in our hospital during the same period were selected as the healthy group.All participants underwent electromyography examination.The motor nerve conduction velocity(MNCV)and sensory nerve conduction velocity(SNCV)of the median nerve,ulnar nerve,and common peroneal nerve were measured and compared among the three groups.Receiver operating characteristic(ROC)curves were plotted to analyze the value of electromyography parameters in diagnosing DPN of T2DM patients.Furthermore,DPN patients were subdivided into the symptomatic group(56 cases)and the asymptomatic group(42 cases)based on the presence of symptoms,and electromyography parameters were detected and compared between the two groups.Results Compared with the control group and the healthy group,the MNCV and SNCV of each nerve in the observation group were lower(P<0.05).ROC curve analysis showed that the area under the curve of the combined use of the MNCV and SNCV of the median,ulnar,and common peroneal nerves in the diagnosis of DPN in T2DM patients was higher than that of each indicator alone(P<0.05).Compared with the asymptomatic group,the MNCV and SNCV of each nerve in the symptomatic group were lower(P<0.05).Conclusions Electromyography can effectively diagnose DPN in T2DM patients and also provide good assessment of disease progression,demonstrating certain application value.
