目的 探讨高压氧治疗仪联合吡拉西坦辅助静脉溶栓在急性缺血性脑梗死(ACI)患者中的应用价值.方法 回顾性分析 2023年 7 月至 2025 年 6月收治的 88 例ACI患者的临床资料,按照治疗方式分为研究组与对照组.对照组在溶栓基础上联合吡拉西坦治疗,研究组在对照组基础上采用高压氧治疗仪治疗.比较两组的神经功能、预后、脑组织血流灌注情况、血清指标、治疗效果.结果 治疗后,研究组的NIHSS评分、mRS评分均低于对照组(P<0.05).治疗后,研究组的CBF、CBV均高于对照组,MTT、Tmax均短于对照组(P<0.05).治疗后,研究组的Hcy、CRP、D-D水平均低于对照组,Ang-1 水平高于对照组(P<0.05).研究组的治疗总有效率高于对照组(P<0.05).结论 高压氧治疗仪联合吡拉西坦辅助静脉溶栓治疗ACI可改善患者的神经功能及预后转归,治疗效果理想,其机制可能与改善脑组织血液灌注及调节Hcy、D-D、CRP、Ang-1表达有关.

Objective To explore the application value of hyperbaric oxygen therapy apparatus combined with piracetam-assisted intravenous thrombolysis in patients with acute ischemic cerebral infarction(ACI).Methods The clinical data of 88 ACI patients from July 2023 to June 2025 were retrospectively analyzed.All patients were divided into the study group and the control group based on the treatment methods.The control group was treated with piracetam combined with intravenous thrombolysis,while the study group was treated with hyperbaric oxygen therapy apparatus on the basis of the control group.The neurological function,prognosis,cerebral blood perfusion status,serum indicators,and treatment effect were compared between the two groups.Results After treatment,the NIHSS score and mRS score of the study group were lower than those of the control group(P<0.05).After treatment,the CBF and CBV of the study group were higher than those of the control group,while the MTT and Tmax were shorter than those of the control group(P<0.05).After treatment,the Hcy,CRP and D-D levels of the study group were lower than those of the control group,while the Ang-1 level was higher than that of the control group(P<0.05).The total effective rate of treatment in the study group was higher than that in the control group(P<0.05).Conclusions Hyperbaric oxygen therapy apparatus combined with piracetam-assisted intravenous thrombolysis in the treatment of ACI can effectively improve patients'neurological function and prognostic outcomes,achieving favorable treatment efficacy.The underlying mechanism may be associated with the improvement of cerebral blood perfusion and the regulation of the expressions of Hcy,D-D,CRP and Ang-1.
