PYCR1在结直肠癌诊断和随访中的价值分析OA
Value of PYCR1 in diagnosis and follow-up of colorectal cancer
目的 探讨吡咯啉-5 羧酸还原酶 1(pyrroline-5-carboxylate reductase 1,PYCR1)在结直肠癌患者血清中的表达情况及其临床价值.方法 采用基因表达谱交互分析(Gene Expression Profiling Interactive Analysis,GEPIA)数据库分析结直肠腺癌组织中PYCR1 的表达情况.收集 2022 年 2 月至 2023 年 12 月就诊于上海中医药大学附属第七人民医院的 133 例血清样本及其相关资料,其中健康体检者 20 例,结直肠腺瘤患者 31 例,结直肠腺癌术前患者 28 例,结直肠腺癌术后无复发患者 30 例,结直肠腺癌术后复发患者 24 例.采用酶联免疫吸附试验检测血清样本中的PYCR1 表达水平.分析血清PYCR1 水平与临床特征和肿瘤标志物的相关性.采用受试者工作特征曲线评估其对结直肠癌诊断和随访的临床价值.结果 GEPIA数据库分析显示,PYCR1 在结肠腺癌和直肠腺癌组织中均高表达(均P<0.05).与健康体检者比较,结直肠腺瘤、结直肠腺癌术前和结直肠腺癌术后复发患者中血清PYCR1 水平均升高(均P<0.05).血清PYCR1 高表达与肿瘤大小和远处转移有关(均P<0.05),与癌胚抗原(carcinoembryonic antigen,CEA)水平呈强正相关(r=0.832,P<0.01),与糖类抗原 19-9(carbohydrate antigen 19-9,CA19-9)(r=0.632)、CA50(r=0.587)和CA242(r=0.574)呈中等正相关(均P<0.01).与CEA和CA19-9 联合检测比较,血清PYCR1 和CEA联合检测在结直肠癌诊断和预测术后复发中具有更优的敏感度和阴性预测值.结论 血清PYCR1 高表达与结直肠肿瘤进展相关,与CEA联合检测对结直肠癌诊断和随访具有一定的临床价值.
Objective To investigate the expression of pyrroline-5-carboxylate reductase 1(PYCR1)in the serum of patients with col-orectal cancer and its clinical value.Methods The expression of PYCR1 in colorectal adenocarcinoma tissues was analyzed in the Gene Expression Profiling Interactive Analysis(GEPIA)database.The serum samples and related data of 133 patients who were admitted to the Seventh People's Hospital of Shanghai University of Traditional Chinese Medicine from February 2022 to December 2023 were collected,including 20 healthy subjects,31 patients with colorectal adenoma,28 pre-operative patients with colorectal adenocarcinoma,30 post-op-erative colorectal adenocarcinoma patients without recurrence,and 24 post-operative colorectal adenocarcinoma patients with recurrence.The expression of PYCR1 in the serum samples was detected by enzyme-linked immunosorbent assay.The correlations between the serum PYCR1 level and the clinical features and tumor markers of the patients were analyzed.The clinical value of serum PYCR1 level in the di-agnosis and follow-up of colorectal cancer was evaluated by receiver operating charateristic curves.Results The GEPIA database analysis showed that PYCR1 was highly expressed in colon adenocarcinoma and rectal adenocarcinoma tissues(both P<0.05).Serum PYCR1 levels were increased in patients with colorectal adenoma,pre-operative colorectal adenocarcinoma,and post-operative recurrence of colorectal adenocarcinoma(all P<0.05).The high level of serum PYCR1 was correlated with tumor size and distant metastasis(both P<0.05),strongly positively correlated with the levels of carcinoembryonic antigen(CEA)(r=0.832,P<0.01),and moderately positively correlated with carbo-hydrate antigen 19-9(CA19-9)(r=0.632,P<0.01),CA50(r=0.587,P<0.01),and CA242(r=0.574,P<0.01).Compared with the combined detection of CEA and CA19-9,the combined detection of serum PYCR1 and CEA had better sensitivity and negative predictive value in the diagnosis of colorectal cancer and prediction of post-operative recurrence.Conclusions The high level of serum PYCR1 is related to the progression of colorectal neoplasms,and the combined detection of serum PYCR1 and CEA has certain clinical value in the diagnosis and follow-up of colorectal cancer.
